The Role
As a Case Manager, you’ll provide community-based support to adults working toward recovery. You’ll connect clients to resources, help build daily living skills, advocate on their behalf, and support their personal recovery goals. Flex hours may be available to best serve client needs.
What You’ll Do
- Provide individual and group case management under clinical supervision
- Support clients through community activities, facility visits, vocational training, and coordination of care, as outlined in each Individual Treatment Plan
- Travel to meet clients in the community using a company vehicle (properly insured)
- Document services accurately and follow all company policies and procedures
- Work with the treatment team and managers to improve the quality of our services
What You’ll Bring
- High school diploma or college degree from an accredited institution
- An Active Ohio Driver's license with a clean driving record
- At least one year of experience in behavioral health or a related field
- Excellent communication skills and working knowledge of behavioral health issues and current therapeutic methods
