Job Description: PRN Physical Therapist (Home Health)Position Summary
The PRN Physical Therapist (PT) provides skilled physical therapy services to patients in their homes in accordance with physician orders, agency policies, and state and federal regulations. The PT evaluates patients, develops individualized treatment plans, implements therapeutic interventions, and collaborates with the interdisciplinary team to maximize patients' functional independence, safety, and quality of life.
Essential Duties and Responsibilities
- Perform comprehensive physical therapy evaluations to determine patients' functional status, mobility limitations, strength, balance, pain levels, and rehabilitation needs.
- Develop individualized plans of care based on physician orders and patient goals.
- Provide skilled physical therapy interventions, including therapeutic exercise, gait training, balance training, transfer training, pain management, and fall prevention strategies.
- Educate patients, families, and caregivers regarding home exercise programs, safety techniques, use of assistive devices, and disease management.
- Assess the home environment and recommend modifications or adaptive equipment to improve patient safety and independence.
- Accurately document evaluations, treatment visits, progress notes, reassessments, and discharge summaries in accordance with agency policies and regulatory requirements.
- Communicate patient progress and any significant changes in condition to the physician and interdisciplinary care team.
- Participate in case conferences and collaborate with nurses, occupational therapists, speech therapists, social workers, and home health aides to coordinate patient care.
- Complete required documentation within established timeframes.
- Maintain compliance with Medicare Conditions of Participation, HIPAA regulations, infection control standards, and agency policies.
- Demonstrate flexibility in scheduling and accepting PRN patient visits based on agency needs.
Qualifications - Graduate of an accredited Physical Therapy program.
- Current, unrestricted Physical Therapist license in the state of practice.
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and current automobile insurance.
Minimum of one year of clinical experience preferred; home health experience preferred - Knowledge of Medicare home health regulations and documentation requirements is preferred.
- Excellent communication, organizational, and time management skills.
- Ability to work independently and manage a flexible schedule.
Physical Requirements
- Ability to travel to patients' homes within the assigned service area.
- Ability to lift, transfer, and assist patients as needed (up to 50 pounds with or without reasonable accommodation).
- Ability to stand, walk, bend, kneel, and perform physical activities required for patient care.
- Ability to safely operate a motor vehicle.
Work Schedule
This is a PRN (as-needed) position. Hours are not guaranteed and are based on patient census and agency staffing needs. The PRN Physical Therapist is expected to maintain flexibility and communicate availability for patient assignments.
