Description:

With the goal of access, quality, efficiencies, and patient satisfaction; provides primary health care to all Hurtt Family Health patients through an integrated and comprehensive team-based, medical home model. Provides direct and indirect clinical supervision of mid-level providers and provides support on complex medical care issues. Participates in the development of mid-level providers, RNs, and medical assistants.

Schedule: This on-call position provides coverage as needed with an expected variable schedule of 8-20 hours per week.

Location: This position may be assigned to our clinics in Tustin, Santa Ana, and Anaheim.

The best candidate for this position:

  • is a Board certified Physician and an accredited Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
  • has 3-5 years of experience as a Physician
  • the ability to to foster a safe environment where people will feel comfortable to share private, pertinent information for effective treatment
  • the ability to diagnose and treat medical issues commonly experienced by homeless persons and uninsured persons
  • understands the psychosocial factors involved in administering health services for homeless and uninsured persons
  • experience in a Federally Qualified Health Center (FQHC) or community health setting (preferred)
  • ability to communicate in Spanish in a healthcare environment (preferred)

What You'll Do:

Job Responsibilities & Duties

· Project and carry out the mission of the clinic to provide high-quality integrated primary care that is compassionate, culturally sensitive, equitable, and trauma-informed, to all patients and families who are eligible to receive services at Hurtt

· Work efficiently with care team members to provide a minimum average of 9 patients per 4-hour sessions and respond to assess the need for medical services for all walk-ins while on duty.

· Maintain good working relationship and communication with all staff, reflecting professionalism in all interactions.

· Required to maintain a flexible schedule which may include coverage of Hurtt clinical sites, evenings and weekends, after hour coverage, mobile outreach clinics and promotional or educational events.

· Provide professional, administrative review of patient labs, studies, specialty reports and medical records and medication refill requests as needed in a timely, efficient manner and direct appropriate follow up care as needed. Respond to telephone calls, and jelly bean (patient portal). Complete appropriate medical forms. Direct support staff to assist in these tasks as appropriate.

· Provide appropriate after hours care as assigned: on call telephone duties.

· Provide oversight of employed and volunteer mid-level providers in a manner consistent with the State of California Laws and Regulations. This includes appropriate chart review while maintaining supervisory accessibility. Chart reviews to be documented.

· Supervision and teaching of Nurse practitioners, Physician Assistants, residents and or students as assigned as applicable

· Provides on-going training for employed mid-level providers to increase their effectiveness and base of knowledge. Provide expertise to midlevel providers as requested.

· Refer for specialty services and ancillary services as appropriate per managed care and DHS guidelines

· Keep abreast of current clinical guidelines, literature and research and complete trainings as needed to implement and update services as mandated by grants, licensing requirement, and contracts. This includes, but not limited to, maintaining CME, DEA, CPR and licensure.

· Ensure timely credentialing by maintaining Physician License, PECOS and DEA License, NPI Number, etc. and submitting proof annually.

· Attend All Staff and Provider meetings and be responsible for all information presented.

· Appropriate utilization/adoption of electronic health record, to include chart documentation and coding for services provided and timely encounter closure within 24 hours. Document all patient encounters thoroughly in the EMR system to meet standards of practice and all required UDS, CQMs and HEDIS measures.

· Comply with Hurtt policies and procedures and regulatory Federal, State and local rules and regulations including public health advisories and CMR reporting.

· Participate in all safety programs which may include assignments to an emergency response team for Disaster Preparedness

· Timely communication with HR and administrative staff to maintain credentialing and audit compliance requirements.

· Participate in PEER review, Hurtt QI programs, and Continuous Quality Improvement, and in assuring quality patient care is maintained.

· Reports and responds to complaints regarding clinical care, resolving any complaint with expediency and in a professional manner in coordination with staff.

· Participates in the formulation of policies and procedures on all aspects of clinical care.

· Lead patient access and efficiency by modeling high effectiveness, training, and assisting other providers in maximizing time and access to care. This includes providing feedback to admin and provider mentorship.

· Perform other duties as assigned.

  • Education: Successful completion of an accredited Medical Doctor program as a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
  • Experience: 4-5 yrs of job related work experience.
  • Job Related Certifications / Licensing: Board certified doctor; Current California Physician license; DEA number; NPI number; enrolled in PAVE

Skills & Qualifications

  • Completion of medical clinical doctor program clinical/residency requirements
  • 3-5 years of experience as a Physician
  • Admitting privileges at various Orange County, CA hospitals
  • Ability to foster a safe environment where people will feel comfortable to share private, pertinent information for effective treatment.
  • Ability to diagnose and treat medical issues commonly experienced by homeless persons and uninsured persons
  • Understanding of the psychosocial factors involved in administering health services for homeless and uninsured persons
  • Preferred: Experience in a Federally Qualified Health Center (FQHC) or community health setting
