Temporary Assignment

Friday Workforce Solutions is seeking a compassionate and experienced Health Services Social Worker to support a respected continuing care retirement community in Black Mountain. This temporary opportunity is ideal for a social worker who is passionate about advocating for older adults and collaborating with interdisciplinary healthcare teams to provide exceptional resident care.

Responsibilities:

Serve as the primary social services contact for skilled nursing, long-term care, and assisted living residents
Complete psychosocial assessments and develop individualized care plans
Collaborate with nursing leadership and interdisciplinary teams to coordinate resident care
Advocate for resident rights and assist with resolving resident and family concerns
Coordinate discharge planning and care transitions with residents, families, and outside agencies
Assist with admissions and provide support to the admissions process as needed
Maintain accurate documentation and ensure compliance with applicable regulations
Perform other duties as assigned to support the healthcare team

Qualifications:

Bachelors degree in Social Work (BSW) required
Minimum of 2 years of experience working with older adults
Valid North Carolina drivers license with a clean driving record
Excellent communication, organization, and problem-solving skills
Ability to work collaboratively in a fast-paced healthcare environment
