COMMUNITY OUTREACH SPECIALIST-GRANTS

The Community Outreach Specialist works closely with patients, medical providers, and social service agencies to promote the overall health and well-being of the community. The role fosters engagement and collaboration between the health system and the communities it serves by providing short-term care coordination, connecting patients and community members to appropriate resources, and offering support and education to improve health outcomes and overall well-being. The Community Outreach Specialist also facilitates coordination of care and services to help individuals access the support they need.

Community Outreach Specialist Essential Job Duties:

  • Community Engagement
    1. Building relationships with individuals, groups, and organizations to foster a positive image of their organization and promote its services.
  • Program Development
    1. Creating and implementing outreach programs, events and initiatives to address specific community needs.
  • Needs assessment
    1. Identifying and understanding the needs of the community to tailor outreach efforts effectively.
  • Relationship Management
    1. Maintaining contact with key stakeholders, including community members, partner organizations, and donors.
  • Communication and Marketing
    1. Developing and distributing outreach materials, such as brochures, flyers, and social media content, to promote the organization’s work.
  • Data Collection and Analysis
    1. Developing and distributing outreach materials, such as brochures, flyers, and social media content, to promote the organization’s work.
  • Event Coordination
    1. Organizing and managing events, presentations, and workshops to engage the community.

Education, Training & Experience

  • Bachelor’s degree in Communications, Marketing or health related field
  • Minimum of three years’ experience of related experience.
  • Community Health Worker Certification preferred
  • Applicants must have a working vehicle to successfully facilitate transportation to/from but not limited to: home visits, community visits, and clinic sites.
