Care Partner in Athens, Georgia at HACS OpCo LLC

We are seeking compassionate, dependable, and caring Care Partners to join our Assisted Living team. The Care Partner provides hands-on assistance and personal support to residents while promoting dignity, independence, comfort, and quality of life.

The ideal candidate enjoys working with older adults, has a positive and patient attitude, and understands the importance of providing respectful, person-centered care.

Responsibilities
Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and personal hygiene.
Assist residents with mobility, transfers, positioning, and ambulation as appropriate.
Provide medication assistance or reminders as permitted by facility policy, training, and applicable regulations.
Respond promptly and respectfully to resident requests and call systems.
Observe residents for changes in condition, behavior, mobility, appetite, or overall well-being and promptly report concerns to the appropriate supervisor.
Assist residents with meals, hydration, and dining as needed.
Encourage residents to participate in activities, social programs, and daily routines.
Provide companionship and emotional support while maintaining appropriate professional boundaries.
Maintain residents' rooms and common areas in a clean, safe, and comfortable condition as assigned.
Follow resident service plans and established care procedures.
Complete required documentation accurately and on time.
Follow infection-control, safety, emergency, and resident-protection procedures.
Maintain resident privacy, dignity, confidentiality, and rights at all times.
Communicate professionally with residents, families, coworkers, supervisors, and other members of the care team.
Attend required training, meetings, and in-service education.
Perform other resident-care duties as assigned.
