Schedule

• Monday- Friday 8am-5pm
• Schedule subject to change as indicated by Practice Administrator
• This is an onsite position

Qualifications

• Medical Assistant or Licensed Practical Nurse (LPN)
• Minimum of 2 years experience in a medical office is strongly preferred

Support staff hired at a 0.50 FTE or greater on or after April 1, 2024, are eligible for our structured retention bonus program:

Paid Maternity/Paternity Leave

After one year of employment and 1,250 hours worked, Eagle provides up to two weeks of paid maternity/paternity leave for the birth or adoption of a child. Leave is prorated based on FTE status.

Key Responsibilities

• Attends required meetings and participates in committees as requested.
• Participates in professional development activities and maintains professional affiliations.
• Functions as an active member of the interdisciplinary care team by communicating patient care needs, quality trends, and outcomes to the provider.
• Completes pre-visit planning by identifying upcoming patients, reviewing charts, identifying gaps in care, and initiating appropriate standing orders.
• Initiates standing orders, laboratory orders, and medication refills as authorized and appropriate.
• Identifies patients eligible for Annual Medicare Wellness Visits (AWV), educates patients on the importance of preventive care, assists with scheduling, conducts AWVs, and refers patients to the provider when appropriate.
• Completes and accurately documents AWV information and other patient care activities in the Electronic Medical Record (EMR).
• Understands and supports system quality improvement initiatives, including Meaningful Use (MU), Accountable Care Organization (ACO), and Patient-Centered Medical Home (PCMH) requirements.
• Reviews provider-level quality improvement data, evaluates the effectiveness of interventions, adjusts plans as needed, and communicates outcomes to the provider and Practice Manager.
• Reviews patient records to identify patients due for screenings, diagnostic tests, vaccines, and immunizations.
• Coordinates with schedulers to arrange appropriate patient appointments.
• Conducts visits to provide appropriate screenings, tests, vaccines, and immunizations within scope of practice and established protocols.
• Documents all patient care, screenings, procedures, and interventions accurately and timely in the EMR.
• Initiates Chronic Care Management (CCM) coordination when indicated.
• Collects and reviews pharmacy reports to assess medication adherence.
• Works collaboratively with patients to identify and address barriers to medication adherence.
• Reviews and reconciles medication lists, comparing patient-reported medications with EMR documentation and making appropriate updates.
• Operates medical equipment safely and appropriately, including blood pressure cuffs, thermometers, needles, syringes, and EKG equipment.
• Maintains clean, organized, and adequately stocked work areas and exam rooms.
• Monitors medical supply inventory and communicates supply needs appropriately.
• Ensures restricted or potentially abusable materials, including medications, needles, syringes, prescription pads, and patient medical records, are secured and accessible only to authorized personnel.
• Follows clinic policies and procedures regarding medication samples, including dispensing only with appropriate provider authorization.
• Disposes of infectious waste in accordance with established training and safety procedures.
• Maintains patient confidentiality and follows all applicable HIPAA requirements related to Protected Health Information (PHI).
• Does not access, disclose, copy, or remove the contents of patient records or reports except as required to perform assigned job responsibilities.
• Demonstrates respect, courtesy, and advocacy in all interactions with patients, coworkers, providers, and visitors.
• Maintains a positive, professional, and team-oriented attitude and willingly assists coworkers and the practice as needed.
