THE POSITION

In this vital role, you will work closely with our nursing staff. Assists in meeting the psychosocial needs of patients in order for patients and families to achieve their optimal level of health. Responsible for providing discharge planning services based on assessment and treatment team recommendation. Meets the post-discharge needs of the patient and family through linkage with community resources including long-term care facilities, assisted living facilities, Home Health agencies, equipment companies, etc. Your commitment to compassionate care will have a direct impact on the lives of those we serve, fostering a healing and supportive environment. Must have BLS/CPR through the American Heart Association.
