Charge Nurse LPN

Position Summary

The Charge Nurse LPN provides and coordinates comprehensive nursing care for residents, ensuring adherence to facility policies and applicable state and federal regulations. This role collaborates with nursing leadership and clinical staff to meet established care goals, supervises nursing personnel, and supports daily clinical operations. The Charge Nurse LPN also performs Certified Nurse Aide duties as needed.

Essential Duties and Responsibilities
  • Assess resident health status, implement care interventions, and develop individualized Care Plans. Document all observations accurately in the medical record system (PCC).

  • Conduct independent and physician-assisted rounds to monitor resident conditions, evaluate treatment effectiveness, and initiate necessary nursing interventions.

  • Deliver preventative, supportive, maintenance, and rehabilitative care addressing physical and psychosocial needs of residents.

  • Perform clinical procedures including dressing changes, wound care, Foley catheter irrigation, IV therapy, tube feedings, chest tube management, and peritoneal dialysis in accordance with protocols.

  • Coordinate availability and restocking of necessary supplies and equipment to support resident care.

  • Schedule and document resident appointments with physicians, therapists, and consultants.

  • Supervise and assist CNAs in providing personal care services such as bathing, feeding, and ambulation to ensure resident comfort.

  • Receive shift reports from outgoing Charge Nurse and perform timely resident rounds as required.

  • Administer medications following legal and facility guidelines, promptly reporting any adverse reactions or contraindications.

  • Communicate medication, treatment plan changes, and resident condition updates to residents and their responsible parties.

  • Manage emergency care situations, coordinating communication with healthcare providers, family members, and administration.

  • Conduct comprehensive nursing assessments related to resident condition changes and scheduled programs.

  • Facilitate resident admissions, transfers, and discharges, ensuring necessary supplies and documentation are complete and accurate.

  • Participate in quality improvement initiatives as directed by nursing leadership.

  • Monitor and report incident, accident, and infection rates, recommending corrective measures when necessary.

  • Perform weekly skin assessments to identify pressure injuries or other conditions and report findings to the Director of Nursing.

  • Promptly report resident and staff complaints, unusual incidents, and any suspected abuse or neglect to appropriate authorities.

  • Support regulatory compliance by participating in inspections and preparing responses.

  • Uphold residents rights, including freedom from restraints and abuse, and ensure proper reporting of any violations.

  • Maintain professional conduct, adhere to facility policies, and serve as a role model for staff.

  • Participate in continuing education and professional development.

  • Promote safety and infection control practices to protect residents and staff.

  • Ensure proper equipment use and report maintenance issues promptly.

  • Adhere to all relevant legal standards including the Nurse Practice Act.

  • Attend required meetings, trainings, and in-service sessions.

  • Perform other duties as assigned.

Minimum Qualifications
  • Current Licensed Practical Nurse (LPN) licensure in the State of Michigan (or applicable jurisdiction).

  • Knowledge of nursing practices, resident care, and clinical procedures.

  • Ability to assess, plan, implement, and evaluate resident care.

  • Strong communication, organizational, and supervisory skills.

  • Commitment to resident dignity, rights, and quality of care.

Physical Requirements
  • Ability to lift, transfer, and reposition residents safely.

  • Prolonged standing, walking, bending, and physical activity as required.

  • Ability to respond quickly and effectively in emergencies.

Work Environment
  • Long-term care facility or clinical environment with exposure to medical conditions and infection risks.

  • Requires adherence to infection control and personal protective equipment (PPE) protocols.
