Position Summary:
We are seeking a Full-Time LPN for 2nd Shift. The ideal candidate will provide high-quality resident care, supervise and support our PCA staff, and promote resident safety, comfort, and independence with professionalism and compassion.
Schedule: Full Time 2nd Shift with some weekend and holiday availability required
RESPONSIBILITIES:
RESIDENT CARE
• Assist residents with personal care (bathing, grooming, dressing, toileting, mobility)
• Provide companionship and emotional support
• Administer medications and monitor resident health status
• Document and report resident care, behaviors, and changes in condition
• Ensure a safe, clean, and respectful environment
• Respond promptly to resident requests and needs
SUPERVISORY & LEADERSHIP RESPONSIBILITIES
• Provide on-shift supervision for PCA staff, ensuring adherence to care plans and facility standards
• Offer guidance, support, and real-time feedback to direct care staff
• Oversee task completion, quality of care, and workflow throughout the shift
• Serve as the primary clinical resource for PCAs and HHAs
• Communicate effectively with management, RN, case managers, and other team members
• Ensure compliance with state regulations, facility policies, and best practices
QUALIFICATIONS:
• Current and valid LPN license in the State of New York
• Previous experience in senior care, assisted living, or a supervisory role preferred
• Compassionate, reliable, and patient
• Strong communication, leadership, and problem-solving skills
• Ability to work independently and as part of a team
• Must be able to meet the physical demands of the role (standing, bending, lifting up to 50 lbs)
