MEDICAL SOCIAL WORKER 2D ACUTE REHAB
2D Acute Rehab (2DAR) is a 15 bed rehab unit that cares for patients with hip fractures, CVAs (Cerebrovascular Accident) and other debilities. Patients participate in 3 hours of therapy 5 days a week. They welcome new grads, and have no call or rotating shifts.

POSITION SUMMARY:
The Medical Social Worker oversees the discharge planning process for the high risk patients of Southwest General Health Center. In this process, they assimilate individual, social, medical, and physical information. The goal is to develop a discharge plan that is timely and provides high quality social work services. This position requires an awareness of the community hospital based resources essential for post-hospital planning. Scope of practice includes patients in the age range of 1 to 100+ years of age. The Medical Social Worker must be able to demonstrate the knowledge and skills necessary to provide services appropriate to age groups according to specific chronological age, and/or psychosocial maturity.
MINIMUM QUALIFICATIONS:
Education:
Bachelor’s degree required, Master’s preferred.
Required length of experience:
Two (2) years case work experience required (healthcare, family and children, or mental health).
Healthcare experience preferred.
Required licensure, certification, and/or registry:
Current license by the Ohio State Board as a Licensed Social Worker is required.
