Speech Therapist Full time position or PRN
JOB SUMMARY
Evaluates and treats residents and patients with temporary or long-term speech, hearing, swallowing, and cognitive disabilities in compliance with state and federal guidelines.
- Initiates preventive and rehabilitative therapy procedures as appropriate for the patient s care and safety.
- Develops and revises plan of care and discharge plan in conjunction with patient, family, physician, and care team to assist patient in reaching or maintaining a maximum level of independence.
- Evaluates the patient’s response to therapy services, assesses service needs on an ongoing basis, arranges additional therapy services required, and obtains physician s orders for such services.
- Educates the patient, family, and care team members regarding disease process, self-care techniques and prevention strategies.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Screens and evaluates Resident and develops appropriate care plan.
- Under a physician's direction, provides direct patient / resident treatment based on established plan of care.
- Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.
- Administers, scores, and interprets specialized hearing and speech tests.
- Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.
- Completes required forms and documents in accordance with company policy and state and/or federal regulations.
- Maintains patient records in an accurate and timely manner.
- Devises special tools and treatments for the specific needs of patients.
- Other duties as assigned.
Documentation:
- Documentation demonstrates compliance with Medicare, Medicaid, and other regulatory requirements.
- Documentation reflects notification of physician when patient’s status changes and revision to the plan of care as appropriate.
- Documentation of visits reflects the patient’s response to care and assessment of psycho-social needs through out the course of care.
- Clinical documentation with error rate less than 5%
- Documentation reflects complete assessment/reassessment of patient and includes goals, plan, and evidence of discharge planning.
JOB QUALIFICATIONS
- Graduate of an accredited School of Physical Therapy/ Occupational Therapy/Speech Pathology with a Baccalaureate or
- Master s Degree.
- Must have a current licensure by the South Carolina State Board of the specified therapy and a valid CPR certification.
- Licensed in Speech Language Pathology as required by state law.
- Certification in clinical competency.
- One-year clinical experience preferred
OTHER REQUIREMENTS
Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident's protected health information. Monitors and ensures compliance with company policies, procedures and state and federal law.
