About This Position
Advanced Practice Provider — Boulder Heart
Boulder Community Health, Boulder, CO 80303
$52.48 - $78.72 an hour — Full-time days, 40 hours per week — 10-hour days.
On-call requirement — No weekends
Boulder Community Health is seeking an Advanced Practice Provider for Boulder Heart who provides primary health care and performs selective medical services under the direction of a physician.

Qualifications
•Current Colorado Physician Assistant or Nurse Practitioner License.
•Must be credentialed by the Boulder Community Hospital Medical Staff Office prior to hire date.
•BLS certification is required by the end of the conditional period.
•ACLS certification required within 6 months of hire.
•Experience in multi-specialty clinic preferred
Responsibilities
•Obtains complete medical history and physical data on patients. Reviews patient records.
•Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.
•Performs complete physical exam and records findings. Establishes presumptive diagnosis.
•Orders appropriate laboratory and diagnostic procedures.
•Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
•Prepares written prescription orders for drugs.
•Assists in transition of care from inpatient to outpatient settings.
•Completes billing for hospital and clinic visits in a timely manner.
•Interviews and advises patients regarding health and illness prevention.
•Instructs patient and family regarding medications and treatment instructions. Provides patient education.
•Assists physicians by recording patient progress notes. Issue diagnostic orders and transcribes orders.
