Responsibilities:

• Receives and gives report on resident status at the beginning and end of each shift and as needed. Notifies the clinical team of any changes in resident physical and/or mental status.
• Provides personal care to residents per their individualized plan of care.
• Completes prevention and restorative care including, but not limited to, ambulation, transfers, bathing, grooming, and all other ADL care.
• Documents all pertinent information regarding care as assigned.
• Assists in turning, positioning, and repositioning of residents as outlined in the resident's service plan.
• Communicates effectively with the residents, families, visitors, members of the health care team, and other disciplines

Requirements:

• NJ Certified Home Health Aide (CHHA) certification must be current and active
• Ability to work in a collaborative work environment while being accountable for individual goals
• Ability to respond to and work with a mature population, contributing to a warm, caring and supportive environment for the enjoyment of the residents/clients and to promote this philosophy to all team members
• Must be available to work weekends, as scheduled
