What You’ll Do
- Conduct one-on-one patient care and education in the home setting
- Perform patient assessments and collaborate with the care team on plans of care
- Maintain accurate, timely documentation of patient visits
- Communicate regularly with clinical management and care teams

Requirements:What We’re Looking For
- Graduate of an accredited RN program
- Current Georgia RN license
- CPR certification
- Reliable transportation, valid driver’s license, and insurance
- Excellent communication, organization, and time-management skills
- Initiative, attention to detail, and ability to multi-task
- Home health / OASIS experience preferred but not required – we provide training!
