Job Description
We are hiring for a Registered Nurse (RN) with strong knowledge of Medicaid Waiver (CCSP, SOURCE, ICWP, GAPP, COMP, NOW) Programs and the Rules and regulations that govern Home Care Services. Candidate must have experience in Behavioral Health Services as well as Intellectual and Developmental Disability Services.
Description: The role is highly autonomous and involves a mix of clinical, educational, and administrative duties:
Travel to clients’ homes to conduct initial assessments of the clients as well as supervisory visits that include head-to-toe physical and psychosocial assessments at every visit.
Administering medicine and medication management
Inspect wounds and change dressings
Assist with personal grooming and hygiene tasks
Test for things like infection, weakness, and bedsores
Developing and updating individualized care plans in collaboration with case managers and other team members.
Maintaining accurate, real-time clinical records and reporting status changes to the case managers
Overseeing the work of home health aides and LPNs (for RNs).
Teaching clients, caregivers and family members how to manage their condition, use medical equipment, and recognize warning signs
Providing companionship and psychological support to patients and families, especially during chronic or terminal illnesses.
Help educate caregivers and family members on how to care for the patient
Monitor clients and keep their case managers in the loop
Key Skills & Requirements:
Must hold an active Registered Nurse (RN)
Minimum of 2 years’ clinical experience in Home Care Services
Ability to work independently
Ability to work in varied and sometimes unpredictable home environments while maintaining high standards of care
Must have excellent clinical judgment to handle emergencies without immediate on-site backup
Strong problem-solving and analytical skills
Great interpersonal skills
Excellent written and verbal communication
Excellent documentation of clinical records
Ability to empathize with patients
Able to provide physical and emotional support
Active listening skills
Up-to-date knowledge of medical practices and equipment
