Job Description
Social Services Coordinator

Care Coordination
- Assess and identify social, emotional, financial, personal, and medical needs to ensure an appropriate discharge plan.
- Coordinate with provider and care coordinator to ensure an appropriate level of care is obtained post discharge.
- Involves the patient/significant other and family in the discharge planning process.
- Assist patients and families with Advance Directives
- Maintain current knowledge of insurance requirements and guidelines
- Assess and identify appropriate APS/DFACS referrals.
Communication
- Effectively communicate with the Care Coordinator, primary/charge nurse, and providers concerning the immediate and post-discharge needs of the patient.
- Participate with an Interdisciplinary Team to ensure seamless discharge.
- Communicates with the Director of Care Coordination about any outliers, concerns, or quality issues.
Documentation
- Ensures complete documentation of findings, plan of care, and outcomes for case management consultants.
- Ensures completion of required documentation for post discharge needs (DMA6, Level 1 and Level 2).
- Ensures IM notice is provided to all Medicare patients within 48 hours of discharge.
Professionalism/Teamwork
- Establishes and maintains a positive professional relationship with representatives from outside agencies.
- Attends employee forums, department staff meetings and in-service.
- Strives to grow professionally by acquiring education specific to the position.
Quality
- Performs high quality care based on hospital and department quality measures.
- Other job duties as assigned.
Education:
- High School or higher
License, Certification, ETC:
- Current LPN license in the State of Georgia, preferred
Exrerience:
Minimum
- 1 year Social Services experience
Preferred
- 3 years Social Services experience
