Job Description

Key Responsibilities
- Coordinate and execute medical records requests from healthcare providers, facilities, and other relevant entities.
- Communicate with facilities, providers, and clients to obtain necessary authorizations, documentation, and clarifying information.
- Track, follow up, and expedite outstanding requests to ensure records are obtained within case timelines.
- Review received medical records to confirm accuracy, completeness, and relevance to the client’s case.
- Identify and summarize key diagnoses, treatments, medications, and other pertinent information for legal review.
- Maintain meticulous documentation of all requests, communications, and records in the case management system.
- Ensure compliance with HIPAA and all applicable privacy laws in the handling of client information.
- Collaborate with attorneys, paralegals, and other case team members to meet deadlines and case objectives.
- Report progress and any obstacles to supervisors promptly to ensure timely resolution.
- Timeliness and accuracy of medical records retrieval.
- Completeness of records provided to case teams.
- Compliance with privacy and legal guidelines.
- Positive collaboration with legal teams, providers, and clients.

Qualifications
- Minimum 1 year of experience requesting and reviewing medical records, preferably in a legal or healthcare setting.
- High school diploma or equivalent required. Associate degree or certification in Health Information Management, Medical Assisting, Medical Records, Medical Billing & Coding, Paralegal Studies, or a related field preferred.
- Working knowledge of medical terminology, diagnoses, treatments, and medications preferred.
- Proficiency with Case Management systems, Customer Relationship Management (CRM) platforms, or comparable systems.
- Proficiency with standard office applications (e.g., Microsoft Office and/or Google Workspace).
- Strong organizational and time management skills, with the ability to manage multiple requests simultaneously.
- Commitment to accuracy, confidentiality, and professional integrity.
- Excellent verbal and written communication skills.
Typical Work Environment/Conditions: Cubicle, air conditioned, moderate noise, extended phone and computer usage.
Essential Functions/Physical Requirements: Ability to sit for long periods of time, speak on the phone using a headset, extended computer and keyboard use.
