Registered Nurse (RN) - Adult Family Home Clinical Oversight (Part-Time) in Madison, Wisconsin at United Care LLC

United Care LLC is seeking a compassionate, licensed Registered Nurse (RN) to provide part-time clinical oversight and nursing support services for our licensed Adult Family Homes (AFHs) in Wisconsin. The RN will support resident health and safety through medication management oversight, staff education, nurse delegation activities, clinical consultation, and regulatory compliance assistance in accordance with applicable Wisconsin laws and regulations.

This position requires travel between company-operated Adult Family Home locations and the maintenance of accurate clinical documentation to support quality care and licensing compliance.

Position Summary

The Registered Nurse serves as a clinical resource to caregivers and management by providing nursing oversight, conducting medication reviews, educating staff, performing nursing assessments within the RN scope of practice, administering physician-ordered injections as appropriate, and supporting compliance with Wisconsin Adult Family Home regulations and nursing practice standards.

Essential Duties and Responsibilities

Medication Management Oversight

• Conduct routine reviews of medication administration practices, including medication storage, labeling, documentation, and disposal procedures.
• Review Medication Administration Records (MARs) for accuracy, completeness, and compliance with physician orders.
• Identify and communicate medication discrepancies, omissions, or concerns to the appropriate staff and Administrator.
• Assist in ensuring medication records reflect current physician orders and prescribed treatments.
• Provide recommendations for medication management process improvements.

Staff Education and Nurse Delegation

• Train and evaluate caregivers on resident care tasks consistent with Wisconsin Adult Family Home regulations and applicable nursing standards.
• Assess whether nursing tasks may be appropriately delegated to qualified unlicensed personnel in accordance with Wisconsin law and professional nursing standards.
• Provide competency-based instruction, supervision, and periodic reassessment of delegated tasks.
• Maintain documentation of training, competency evaluations, and delegation activities as required.
• Monitor delegated tasks and provide additional education or support when resident needs or staff competencies change.

Examples of delegated tasks, when permitted by law and appropriate for the resident and caregiver, may include:

• Blood glucose monitoring
• Oral and topical medication administration
• Application of prescribed topical treatments
• Nebulizer treatments and oxygen equipment assistance
• Enteral feeding support
• Routine ostomy care
• Non-sterile dressing changes
• Other tasks authorized under applicable regulations and nursing standards

Clinical Consultation and Resident Care Support

• Serve as a clinical resource for caregivers regarding resident health concerns, changes in condition, and care planning.
• Review and assess health-related incidents, including falls, injuries, medication variances, and significant changes in condition.
• Provide recommendations regarding appropriate follow-up care and medical referrals.
• Collaborate with physicians, pharmacies, hospice providers, home health agencies, and other healthcare professionals as needed to support resident care.
• Participate in quality assurance and risk management activities.

Injection Administration

• Administer prescribed injections and other nursing services that require licensed nursing personnel and cannot be delegated under applicable regulations.
• Coordinate scheduling of nursing visits to ensure timely completion of ordered treatments.
• Document nursing services provided in accordance with company policy and professional standards.

Documentation and Regulatory Compliance

• Maintain accurate, complete, and timely clinical records.
• Support compliance with Wisconsin Adult Family Home regulations and other applicable federal, state, and local requirements.
• Assist in preparation for licensing reviews, audits, surveys, and quality assurance activities.
• Stay informed of changes in nursing regulations and industry best practices applicable to Adult Family Homes.

• Current, active, and unrestricted Wisconsin Registered Nurse (RN) license.
• Minimum of two (2) years of nursing experience.
• Ability to exercise sound clinical judgment and work independently.
• Strong verbal, written, organizational, and documentation skills.
• Valid driver's license, reliable transportation, and ability to travel between company locations.
• Ability to successfully complete all background screening requirements applicable to the position.

Preferred

• Experience in geriatrics, long-term care, assisted living, home health, hospice, or community-based residential services.
• Knowledge of Wisconsin Adult Family Home regulations and nurse delegation practices.
• Experience training and supervising unlicensed caregiving staff.

Schedule

• Approximately 4 hours per week with flexible scheduling.
• Additional hours may be available based on resident needs, consultation requests, training requirements, or operational demands.

Compensation

Compensation will be based on qualifications, experience, and credentials.

Physical Requirements

• Ability to travel between work locations.
• Ability to assist with resident assessments and observe resident care activities.
• Ability to sit, stand, walk, bend, and lift as necessary to perform essential job functions.
