Job Description
Avery's House is expanding services with a specialized residential mental health program for neurodivergent adolescents, providing individualized, strengths-based, and trauma-informed care. Under the direction of the Clinical Director, the Case Manager provides direct patient support while serving as a mentor, advocate, and positive role model. This role helps maintain a safe, structured treatment environment and collaborates with the multidisciplinary team to deliver high-quality, patient-centered care.
Essential Functions and Responsibilities
Case Management needsDischarge planningCoordinating and collaborating with the treatment teamCommunication with team members the status of clients in verbal and written formCommunicate with the school and obtain online schooling informationCommunicate with the school regarding IEP'sBe present during schoolwork time, monitoring electronic use, and ensuring schoolwork is being doneReviewing classroom or curriculum topics and assignmentsProviding clients with positive and constructive feedbackBuilding meaningful connections with clients from diverse backgroundsAssisting clients with homework, projects, test preparation, papers, research, and other academic tasksMaintain records and related documentation. Produces reports, correspondence and performs case management dutiesTransport clients safely as neededParticipate in documentation in electronic health recordMaintain client confidentiality HIPAA complianceManage crisis situations using Voluntary Time-Out and verbal de-escalation interventionsOther tasks as assignedTherapeutic Milieu Responsibilities
Supervising and engaging clients throughout the shift. Providing continuous support within the therapeutic milieu. Assisting with daily routines, schedules, transitions, and activities. Participating in groups, community meetings, and therapeutic programming as appropriate.Supporting clients with activities of daily living (ADLs) while promoting independence.Supporting clients during meals, hygiene routines, school activities, recreation, and community outings. Utilizing de-escalation, co-regulation, redirection, and crisis intervention techniques. Implementing individualized behavioral support plans under the direction of the clinical team. Maintaining awareness of client whereabouts and supervision requirements. Conducting routine safety and environmental checks. Transporting clients to medical appointments, community outings, or other approved activities as assigned. Assisting with room searches, contraband checks, belongings inventory, and safety procedures according to facility policy. Supporting clients during difficult emotional or behavioral situations while maintaining a calm, therapeutic presence.Collaborating closely with clinical assistants, therapists, case managers, providers, and leadership to ensure continuity of care.Neurodivergent Client Support
Demonstrate understanding of autism spectrum disorder, sensory processing differences, developmental disabilities, and executive functioning challenges.Utilize neurodiversity-affirming and trauma-informed approaches when interacting with clients.Recognize sensory sensitivities and modify communication and environmental factors when appropriate. Support individualized coping strategies, visual supports, structured routines, and sensory regulation interventions. Promotes client dignity, autonomy, and strength-based care.Minimum Qualifications
High School Diploma or equivalent preferred (not required)Current and valid Arizona Fingerprint Clearance CardIf it is a new application, can be submitted and in-processPreferred experience: 1 year in a behavioral healthcare, public service, customer service or salesValid Arizona Driver’s License in good standingDriving Record must permit insurability with the agency’s carrierCurrent CPR/First Aid Certification Participate in supervision with a BHP at least once every two weeksAbility to gain knowledge of:autism spectrum disorder, sensory processing differences, developmental disabilities, and executive functioning challenges.Verbal de-escalation of clientUse of EMRJob Type: Full-time
Pay: Starting at $45,000 annually
