Job Description

Program Director

The Program Director is responsible for the daily operation of the congregate treatment program and for insuring that program activities are designed to effect the lasting rehabilitation of residents and that the program operates in full compliance with Part 595 of the Mental Hygiene Law, organization requirements, Medicaid standards and requirements, and Psychiatric Rehabilitation. The Program Director is responsible for having knowledge of mental illness, substance abuse, restorative services and HIPAA.

The Program Director reports and is supervised by the Executive Director.

This is an exempt position.

Responsibilities include, but are not limited to:

  1. Recruit, hire, evaluate and discipline including terminate as necessary staff of HOGAR ARIEL, Congregate Treatment Program. Obtain written verification of employment and education qualifications and references prior to offering any individual a position in the housing program. Ensure that written verification is maintained on file and available for review by executive director or her designee. Written verification shall be filled in the personnel file of hired individuals.
  2. Ensure that the qualifications of individuals recruited for the housing program meet the needs of the program and part 595, and are relevant for responsibilities of the employment positions for which the individual is applying and is in keeping with established qualifications for each employment position.
  3. Ensure that staff employment schedules coincide with the supportive service needs and availability of residents to participate in the housing program and Part 595 of the Mental Hygiene Law. It is understood that the housing program is a 7 day, 24 hours program and that at least 2 staff persons are to be on duty at all times. It is also understood that direct care staff will be assigned to work evenings, weekends and on holidays. Staff schedules shall vary and change to meet the needs of residents.
  4. Develop and maintain a monthly on call beeper coverage schedule to handle resident crisis and emergencies. The schedule shall include managerial, supervisory and direct care personnel, be posted in the program office and distributed to all residents and staff and the executive director in advance of each month. The schedule shall include month, date, and hours of coverage, primary staff person (direct cave) with beeper number and back-up staff person (supervisory and beeper number.)
  5. Participate in the orientation of newly hired personnel.
  6. Participate in the timely intake and admission process of applicants for the admission to the housing program and in discharge planning of program participants
  7. Physically able to walk up and down stairs
  8. Ensure that supportive services are delivered to residents in accordance with their needs, goals and desires as included in individual service plans. Also ensure that supportive services are delivered to residents in emergency situations as necessary
  9. May assist President/CEO in establishing and maintaining linkage including written linage agreements with locally based support service providers. Assist the President/CEO in review of written linkage agreements at least annual to ensure agreements as necessary
  10. Supervise and direct staff of the housing program including regular individual and Group supervisory meetings; on site observation of staff performance; monitoring the whereabouts of program staff through review of individual and daily destination forms, travel forms and resident case records. Maintain minutes of all supervisory meetings.
  11. Respond to resident crises, emergencies and other situations that require immediate staff involvement and action
  12. Participate in case conferences with resident and program staff.
  13. May develop, maintain and participate in ongoing staff in services training including development of annual in service training schedule that indicate topics, dates of training and staff attending (to attend) in service training. Maintain annual (July 1st to June 30th) in service training cirrocumuli that is to be distributed to staff and executive director prior to (every) July 1st. Annual in service training schedules and amendments as necessary are to maintained in a separate file in the program office with copy provided to President/CEO. In service training topics shall include but not be limited to understanding mental illness and substance abuse, HIPAA, team work, resident nee assessment, service plan development and review , resident functional assessment, confidentiality , legal issues and laws infection control and OSHA guidelines, advocacy, Part 595 and Medicaid requirements including restorative services, CPR, First Aid and Heimlich Maneuver. Documentation of staff participation in in-service education to be filed in each staff person’s personnel record.
  14. Ensure the timely assessment of residents, the timely development and review of individual resident service plans and the timely completion of progress notes.
  15. Conduct regular quality assurance and utilization review of individual resident case records in order to ensure compliance with OMH and Medicaid requirements and an assessment of organization’s meetings of its program objectives as included in the program ‘s license application (CON). A written summary of quality assurance and utilization review findings is to be forwarded to staff members providing supportive services to the resident. Where necessary, a copy of written findings is to maintained in the employee’s personnel file.
  16. May conduct regular reviews of resident rooms and common areas with the involvement of program staff including maintenance personnel. A written summary of findings is to maintained on site. The report should address corrective action(s) as needed.
  17. Work with the organization’s consulting firm as required by the President/CEO to ensure compliance with all requirements. Provide the organization’s consulting firm with information and data as requested
  18. Monitor staff time cards to monitor lateness and early departure as well as the whereabouts of staff and ensure that staff punches in and out as required. Indicate authorized lateness and /or early departure on individual staff time cards.
  19. Prepare bi-weekly time sheets.
  20. Maintain statistical data and other information as required by OHM and the President/CEO
  21. Maintain a current inventory of all program supplies and equipment in accordance with the organization’s inventory control system. Recommend ordering to the President/CEO.
  22. Attend conferences, seminars and in-service training as required by the President/CEO.
  23. May Represent the President/CEO at meetings.
  24. Insure the confidentiality of residents at all times.
  25. Review Admission Notes, Service Plans, Service Plan Reviews and Progress Notes for completeness, compliance with the resident’s needs, strengths, desires and goals and compliance with OMH and Medicaid requirements. Act as the QMHS in the absence of the Supervisor/Case Manager.
  26. May attend meeting of the Board of Directors.
  27. Ensure that all reports, statistical data, personnel evaluations, resident case records and program files are current and up-to-date at all times.
  28. Ensure that all governmental, organization and accreditation rules, regulations, guidelines and standards that govern the program and that are related to the program are administered and maintained
  29. Perform related and additional duties and responsibilities as required by the president/CEO
  30. On Call 24/7

Minimum Qualifications:

  • Bachelor Degree in human services related field
  • 3+ years Director experience
  • On Call 24/7
