Job Description

Job Summary:

The Admissions Coordinator is a member of the patient care team within the Department of Home Care and directly accountable to the Patient Clinical Manager. The position requires that the individual be clinically competent and knowledgeable of home health regulations. Responsible for managing the daily workques and reports as assigned. Responsible for completing the intake/referral process, validation of OASIS, creation of the Plan of Care, and ICD 10 coding timely and accurately according to the department policies and guidelines. Assists in orientation of new employees as assigned. Able to handle office phone calls related to patient care. Performs other job duties as assigned. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. Able to act as Patient Clinical Manager in absence of the PCM

Performance Expectations

  • Demonstrate ability to perform charting and manage daily workques in the EMR ensuring documentation meets all state and federal regulations

Qualifications:

Education/Skills

Graduate of an accredited non-online school of nursing required.

Work Experience:

Minimum one year nursing experience required.

Certification/Licensure-DUE UPON HIRE

  • Licensed RN able to practice within the State of MS
  • Valid Drivers License
  • Proof of Automobile Insurance

Additional Certification/Licensure - Obtained based on required timeframe below

  • Basic Life Support

Within 30 Days of Employment

Must have ability to assess, plan, and coordinate patient care, work independently, carry out orders accurately, demonstrate initiative, prioritize, have good communication skills, and ability to perform as a team member, and function under stress.
