Piedmont Health Partners is a local Clinically Integrated Network owned by AnMed Health and comprised of employed and independent healthcare providers. The role of Health Promoter establishes and maintains collaborative relationships with Health Partner teammates, physicians, other health care providers, patients and their families, to achieve desired patient outcomes throughout the continuum of care. The goal is to help PHP achieve the "Quadruple Aim" of lowering healthcare costs, improving patient satisfaction, increasing quality of care and enhancing the lives of healthcare providers in the network. Health Promoters work in a variety of settings and deal with a variety of issues both clinical and nonclinical.
Duties & Responsibilities
- Coach individuals about how to use the health care and social service systems.
- Facilitate communication between health and social service providers and individuals.
- Facilitate continuity of care through education, support and reinforcement.
- Identify potential enrollees and assist them with applications for programs such as health insurance and public assistance.
- Inform individuals about and connect them with health and social service resources they may need.
- Gathers data from individuals and the community as appropriate that can be used to inform decision makers about population health needs and possible ways to address those needs.
- Supports program evaluation as requested.
- Identify continuing professional development needs and ways to fulfill them.
- Address transport needs for healthcare appointments, as needed, to include Uber/Lyft, and/or local transportation resources.
- Performs other duties by authorized personnel.
Qualifications
- Certified Medical Assistant.
- Two years of healthcare experience.
- Willingness to become a Certified Health Coach if not already.
- Comfortable in a variety of settings meeting patients and working with a team.
- Familiarity with EHR, Microsoft Word, and/or other similar computer software.
- Willingness to learn new skills and function in a non-traditional CMA role.
- Valid South Carolina Driver's License.
- Must be willing to use personal transportation with mileage reimbursed.
