Supervisor - Dual CCS/CLTS

Full-Time | Milwaukee, WI | Approximately 40 hours/week

Pay Range: $50,332 - $75,498


We are seeking a Supervisor - Dual CCS/CLTS to lead a team of approximately 8–12 Care Coordinators serving youth with developmental, physical, behavioral health, and/or mental health needs.

What You'll Do

  • Supervise, coach, and develop a team of 8–12 Care Coordinators.
  • Champion family-centered, trauma-informed, strength-based, and culturally responsive care.
  • Oversee CCS and CLTS care coordination services and support staff with complex cases.
  • Train and onboard new Care Coordinators.
  • Monitor documentation, service utilization, authorizations, progress notes, and required reviews.
  • Conduct quality assurance audits and ensure compliance with Milwaukee County, CCS/CLTS, and DHS 36 requirements.
  • Maintain strong partnerships with Milwaukee County CCS/CYFS, Wraparound Milwaukee, families, and community providers.
  • Participate in program meetings, quality improvement activities, and external audits.
  • Respond to critical incidents and provide leadership during challenging situations.
  • Provide Care Coordinator coverage when needed.


What We're Looking For

Required:

  • Associate's or Bachelor's degree in human services, social work, psychology, education, sociology, or related field plus 1 year of CCS/CLTS experience; OR
  • Associate's or Bachelor's degree in another field plus 4 years of experience working with the target population, including 1 year of CCS/CLTS experience.
  • Strong communication, organization, problem-solving, and time-management skills.
  • Ability to work effectively with diverse youth, families, providers, and community partners.
  • Experience with electronic health records and Microsoft Office.
  • Ability to work independently while providing strong team leadership.
  • Valid driver's license and ability to drive for community-based work.

Work Environment

This position combines office, remote/computer, and community-based work. Some visits occur in youth and families' homes and communities. The position averages 40 hours per week, with some evening/weekend flexibility based on program needs.
