Physical Requirements
This position will require driving between multiple sites over defined service areas in Kentucky.
Shift
This position must be willing to work in different environments (both office-based and on the ground), at times best suited to the community (including some evenings and weekends). This includes hours outside of traditional office hours (i.e., 8:00 a.m. – 5:00 p.m. or 9:00 a.m. – 6:00 p.m.).
Job Summary
This position will oversee the Kentucky Area Developmental Districts – Purchase, Pennyrile Green River, Barren River, Lincoln Trail

The Community Health Advocate Manager provides day-to-day leadership and operational oversight of community and mobile health services focused on HIV and HCV prevention, testing, and linkage to care. This role supervises, coaches, and supports Community Health Advocates to deliver high-quality, client-centered, trauma-informed, and culturally responsive services. The supervisor ensures effective outreach, engagement, linkage, referral coordination, and continuity of care while monitoring compliance, performance outcomes, and data quality. Responsibilities include managing daily operations, staff training and development, community partnerships, and quality improvement efforts. The position contributes to program planning, policy, and workflow development, and represents the program in community and public health initiatives to strengthen referral networks, improve retention and treatment adherence, and advance overall program effectiveness.

MS Word, Excel, Access, Outlook and other Windows systems.

Preferred Education/Experience
MA in Public Health, Health Education, Social Work, Social Services, or Human Development highly preferred.
