Job Description
Some jobs pay the bills. Others give you a reason to get out of bed in the morning. This one can do both.
At Mental Health Kokua (MHK), we believe that everyone deserves dignity, compassion, and an opportunity to begin again.
We're looking for a Center Case Manager to join our team at Pūnāwai Rest Stop in Honolulu, where meaningful change often starts with something simple: a conversation.
Every day, individuals experiencing homelessness walk through our doors looking for support, stability, and a path forward.
You could be the person who helps them find it.
WHAT MAKES THIS JOB DIFFERENT?This isn't a typical desk job. You'll work directly with people in our community, helping them navigate real challenges and connect with resources that can make a difference.
You'll be doing more than managing cases. You'll be building trust, restoring connections, and helping people take their next steps toward stability and recovery.
Here's what that looks like:
Be a connection that matters. Engage with guests, listen without judgment, and build relationships grounded in dignity and respect.
Turn obstacles into opportunities. Help individuals access housing, healthcare, benefits, behavioral health services, and community resources.
Create meaningful plans. Complete assessments and develop practical, person-centered goals.
Be steady when things aren't. Provide crisis support, de-escalation, and compassionate guidance during challenging situations.
Work alongside people who care. Collaborate with nurses, fellow case managers, and community partners to coordinate care.
Follow through. Advocate for guests, assist with applications, maintain accurate documentation, and help ensure people don't fall through the cracks.
You don't need to have all the answers. You need the willingness to help people find them.
Our ideal candidate is:
Compassionate, dependable, resourceful, and comfortable working with people from all walks of life.
A strong communicator who can build trust while maintaining professional boundaries.
Organized, adaptable, and able to work in a fast-paced, walk-in community environment.
Comfortable supporting adults experiencing homelessness and behavioral health challenges.
Education: High school diploma or equivalent required. A bachelor's degree in psychology, social work, human services, or a related field is preferred and may be required under certain program contracts.
Additional requirements: Ability to meet background and drug/alcohol screening requirements, obtain necessary CPR/First Aid certifications and TB clearance, lift up to 25 lbs, and work assigned schedules, including rotating on-site coverage.
