Job Description Summary

The Hospice LVN is an integral part of the hospice clinical team. Under the direction of the Director of Nursing, the job responsibilities include the following:

Key Responsibilities:

  1. Patient Care:
    • Provide direct nursing care to hospice patients in their homes, residential care facilities, or hospice facilities.
    • Administer medications and treatments as prescribed by the physician.
    • Monitor patients' conditions and report changes to the RN Case Manager or physician.
    • Assist with personal care tasks such as bathing, dressing, and grooming.
  2. Assessment and Documentation:
    • Conduct patient assessments to evaluate physical, emotional, and psychosocial needs.
    • Maintain accurate, timely, and thorough documentation of patient care and observations in the electronic health record (EHR) system.
    • Participate in the development and implementation of individualized care plans.
  3. Collaboration and Communication:
    • Communicate effectively with patients, families, and caregivers to provide education and support.
    • Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and volunteers, to coordinate care.
    • Attend and participate in team meetings, case conferences, and continuing education programs.
  4. Compliance and Quality Assurance:
    • Adhere to all hospice policies, procedures, and regulatory requirements.
    • Participate in quality improvement initiatives to enhance patient care and outcomes.
    • Ensure compliance with infection control practices and safety standards.

Qualifications:

  • Education: Graduate of an accredited vocational nursing program.
  • Licensure: Current, active LVN license in the state of [State].
  • Experience: Minimum of one year of nursing experience, preferably in hospice, palliative care, home health, or a related field.
  • Skills:
    • Proficient in clinical nursing skills, including pain and symptom management.
    • Strong communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Compassionate, patient-focused approach to care.
    • Proficient in electronic health record (EHR) systems.

Physical Demands:

  • Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
  • Must be able to travel to patient homes and facilities as required.
