Location: Park Forest, IL
Schedule: Full-Time
Work Arrangement: 100% On-Site with 25% travel reimbursed( using personal vehicle)
Starting Pay: $17.50 per hour

As a Community Health Promoter you’ll represent Aunt Martha’s in the community, build relationships with hospitals and community organizations, connect prospective patients with our health centers, and provide hands-on assistance to individuals navigating healthcare services and medical-benefit programs.

Position Overview

The Community Health Promoter conducts community outreach designed to bring new patients to Aunt Martha’s health centers. This position also provides in-person assistance to help patients navigate healthcare services and apply for medical benefits. The role requires someone who can work independently, build strong community relationships, and remain flexible as outreach needs change.

Key ResponsibilitiesCommunity Outreach
  • Execute marketing and outreach activities that result in new patients accessing Aunt Martha’s health centers.
  • Assist with the planning, marketing, and support of special events.
  • Cultivate relationships with community organizations, including schools, churches, human service providers, food pantries, and community medical providers, to generate patient referrals.
  • Develop relationships with Emergency Department and inpatient staff at community hospitals to connect patients with Aunt Martha’s health centers.
  • Engage patients who do not currently have an assigned healthcare provider.
  • Participate in appropriate marketing and community outreach activities.
  • Represent Aunt Martha’s at health fairs and community events.
  • Make presentations to community organizations about Aunt Martha’s healthcare services.
  • Educate prospective patients about available services and Aunt Martha’s model of care.
  • Schedule patient appointments through the electronic health record.

Medical Benefits Enrollment
  • Identify and screen uninsured patients who may qualify for medical benefits.
  • Determine eligibility and assist patients with enrollment in affordable health insurance through Medicaid or the Health Insurance Marketplace.
  • Process online and paper applications for eligible patients.
  • Assist with programs that may include Medicaid, the Illinois Health Insurance Exchange, All Kids, FamilyCare, Moms & Babies, and CountyCare.
  • Process and submit completed applications in a timely manner.
  • Follow up on submitted applications to confirm approval or denial.
  • Research denied applications and assist patients with resubmission when appropriate.
  • Help patients understand Medicaid communications, including selecting a Managed Care Organization (MCO) or Primary Care Provider (PCP).
  • Assist patients with Medicaid Redetermination and Marketplace Open Enrollment.
  • Assist with verification of insurance eligibility and benefits.
  • Maintain accurate daily, weekly, and monthly reports on applications processed.
QualificationsRequired
  • High School Diploma or GED.
  • Strong written and verbal communication skills.
  • Strong organizational skills and attention to detail.
  • Ability to manage multiple priorities and adapt to changing needs.
  • Ability to work independently and as part of a collaborative team.
  • Current driver’s license.
  • Current automobile insurance.
  • Ability to travel as required for community outreach activities.
  • Flexibility to work occasional evenings, weekends, and scheduled holidays as needed.
Preferred
  • Two years of relevant sales or marketing experience preferred.
  • Bilingual English/Spanish skills required
  • Experience with community outreach, healthcare marketing, or patient engagement.
  • Experience presenting to community organizations or participating in health fairs.
  • Experience assisting individuals with Medicaid, insurance, or healthcare-benefit enrollment.
