Physical Therapist (PT) – Full Time, Home Health in Prescott, Arizona at Choice Rehab

As a Full-Time Physical Therapist (PT) in Home Health, you will play a critical role in delivering personalized rehabilitation services to patients within the comfort of their own homes. Your primary objective will be to assess, plan, and implement therapeutic interventions that improve patients' mobility, reduce pain, and enhance their overall quality of life. You will collaborate closely with interdisciplinary healthcare teams, patients, and their families to develop and adjust treatment plans tailored to individual needs and goals. This role requires a compassionate approach, strong clinical expertise, and the ability to work independently while managing a diverse caseload. Ultimately, your work will empower patients to regain independence and achieve optimal functional outcomes in their daily lives.

Minimum Qualifications:

• Valid and unrestricted Physical Therapist license in the state(s) of practice within the United States.
• Graduation from an accredited Physical Therapy program with a Doctor of Physical Therapy (DPT) or equivalent degree.
• Current Basic Life Support (BLS) certification.
• Demonstrated experience or clinical rotations in physical therapy, preferably including home health or outpatient settings.
• Strong knowledge of physical therapy principles, anatomy, physiology, and rehabilitation techniques.

Preferred Qualifications:

• Experience working in home health care or community-based rehabilitation settings.
• Certification in specialized therapy techniques such as wound care, neurological rehabilitation, or geriatrics.
• Familiarity with electronic medical records (EMR) systems and telehealth platforms.
• Excellent communication and interpersonal skills to effectively engage with patients and multidisciplinary teams.
• Ability to manage a flexible schedule and travel independently to patient homes.

Responsibilities:

• Conduct comprehensive patient evaluations to assess physical abilities, limitations, and rehabilitation needs in a home health setting.
• Develop and implement individualized treatment plans based on patient assessments, clinical guidelines, and patient goals.
• Provide hands-on therapy, including therapeutic exercises, manual therapy, and modalities to improve patient function and reduce pain.
• Monitor patient progress regularly, documenting outcomes and modifying treatment plans as necessary to ensure effective care.
• Educate patients and their families on therapeutic techniques, safety measures, and strategies to maintain health and prevent injury.
• Coordinate with physicians, nurses, and other healthcare professionals to ensure integrated and holistic patient care.
• Manage scheduling and travel efficiently to meet patient needs across various home locations within the assigned territory.
