Job Description
DUTIES AND RESPONSIBILITIES:
Under the supervision of the Program Director or designee, in accordance with the standards of the social work profession and requirements of the funding source, responsible for:
Create structured community outreach plans, including project goals and measurements
Perform outreach within the community
Meet with relevant health care, medical and mental health staff and organizations, local clergy, community organizations and other community stakeholders to educate them about Parkchester NORC
Represents Parkchester NORC at community conferences, meetings, fairs and outreach events
Design and create marketing materials
Works with team and community partners to plan monthly trainings to enhance quality of service delivery
Conducts client satisfaction surveys on a semi-annual basis to be analyzed and used for funding sources
Completes required case records, reports, and statistics, utilizing computers, if so required
Performs other duties as assigned.
Closely follow and track all generated referrals to consistently measure and evaluate individual performance and effectiveness, leading to continuous improvement in outreach efforts to maximize referral opportunities.
Directly assist intake activities as necessary, including general qualification screening, documentation collection, benefit education/applications, site tours, home visits, and other follow-up to aid in the enrollment or education of a prospect.
QUALIFICATIONS:
A bachelor’s degree from an accredited college, preferably in a related field including social work, psychology, counseling, gerontology, mental health or marketing
Excellent interpersonal skills, including the ability to communicate effectively and concisely (both orally and in writing)
Experience in public speaking preferred
Must be computer literate, including the ability to learn and use program-specific software
Ability to read, write, speak, and understand English
Desire to work collaboratively and thoughtfully as a part of a committed, interdisciplinary team
Strong attention to detail with the ability to problem solve, prioritize work, work independently, and excel in a high-performance culture
Strong commitment to JASA’s mission and values
An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values have as they relate to serving clients and families.
Written and verbal fluency in a secondary language required.
