JOB SUMMARY
The Care Coordinator serves as a central point of contact for patients, providers, and clinical staff to ensure seamless coordination of care. This role is responsible for scheduling appointments, communicating treatment plans, facilitating follow-up activities, and supporting patients throughout their care journey. The Care Coordinator promotes positive patient experiences and optimal outcomes through proactive communication, organization, and collaboration.
KEY RESPONSIBILITIES
- Coordinate and schedule patient appointments, evaluations, procedures, and follow-up visits.
- Communicate treatment plans, appointment details, and care instructions to patients and caregivers.
- Serve as a liaison between patients, healthcare providers, and other members of the care team to facilitate effective communication.
- Monitor and track patient progress, ensuring recommended follow-up care and services are completed.
- Obtain, organize, and maintain accurate patient information and documentation within the electronic medical record system.
- Respond to patient inquiries and address concerns in a timely, professional, and compassionate manner.
- Coordinate referrals to specialists, community resources, and ancillary services as needed.
- Verify that required authorizations, orders, and supporting documentation are obtained prior to scheduled services.
- Assist patients in understanding their care plans and navigating the healthcare system.
- Collaborate with clinical and administrative teams to identify and resolve barriers to care.
- Maintain confidentiality and comply with all applicable privacy, regulatory, and organizational requirements.

QUALIFICATIONS
- High school diploma or equivalent required; Associate degree in healthcare administration, business, or a related field preferred.
- 1-3 years of experience in healthcare coordination, medical office administration, patient services, or a related role preferred.
- Knowledge of medical terminology and healthcare workflows preferred.
- Experience using electronic medical record (EMR) systems
- Minimum typing speed of 45 WPM; proficiency in Microsoft Word, Excel, and standard office software.
- Strong organizational skills with the ability to manage multiple priorities and deadlines.
- Excellent verbal and written communication skills.
- Demonstrated ability to build rapport with patients and work collaboratively with interdisciplinary teams.
- Strong attention to detail, problem-solving abilities, and customer service orientation.
WORK ENVIRONMENT & PHYSICAL REQUIREMENTS
- This position operates in a healthcare clinic setting with frequent interaction with patients, caregivers, and healthcare professionals in a fast-paced environment.
- Prolonged periods of sitting and working on a computer.
- Frequent communication with patients, providers, and staff in person and by telephone.
- Ability to occasionally lift and move office materials weighing up to 15 pounds.

The pay range for this role is:
16 - 18 USD per hour(SRPT - Rhome)
