Position Summary

The Aultman Registered Nurse (RN) is a professional nurse that provides direct, primary nursing care to patients/residents. The RN delegates and coordinates patient care, based upon a documented assessment of the patient’s/resident’s needs. The RN supervises the care provided by individuals such as licensed practical nurses, patient care technicians/state tested nursing assistants/nurse aides, medical assistants, and etc., if applicable.

Department Summary

The primary purpose of this position is to provide clinical leadership and direction of patient care, provide patient and family teaching, and work cooperatively within the interdisciplinary team in maintaining standards of nursing care within the framework of the Ohio Nurse Practice Act.

Qualifications

  • Graduate of an accredited school of nursing; BSN preferred.
  • Current, active licensure as a Registered Nurse through the Ohio Board of Nursing.
  • Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required.
  • Certification preferred for RNs meeting applicable certification eligibility requirements.
  • Membership in professional nursing organization preferred.

Department Qualifications

  • Graduate of an approved and accredited school of nursing, BSN preferred. Certification preferred for those meeting eligibility requirements. Membership in professional nursing organization preferred.
  • Current State of OH RN license.
  • Two years recent nursing experience in acute, long-term care or homecare/hospice, with at least one in hospice or homecare preferred.
  • Must be able to work independently, have demonstrated patient teaching skills and strong written and verbal communication skills.
  • Current BLS certification
  • Valid OH Drivers License
  • Must have own transportation and provide proof of current liability insurance
  • Basic computer skills

  • RN applies the nursing process of assessment, planning interventions, accurately implements and/or delegates patient care, evaluates that care, and accurately communicates patient information in a timely manner.
  • RN is a patient/resident advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort and satisfaction.
  • Attains knowledge and competency that reflects current nursing practice.
  • Demonstrates quality and effectiveness of nursing practice.
  • Adhere to professional standards and guidelines, including ethical actions, relevant statutes, rules and regulations, during nursing practice.
  • Collaborates with patient, family, and others in the conduct of nursing practice.

  • Provides leadership in the professional practice setting and contributes to the professional development of peers, colleagues and students.

  • Responsible for timely review of emails, department communications and applicable learning assignments and completion as applicable.

Department Responsibilities & Expectations

  • Assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs.
  • Develops an individualized plan of care based on the assessments.
  • Assesses all aspects of the patient’s pain and develops an individualized pain management plan.
  • Anticipates, prevents and treats undesirable symptoms or secondary symptoms.
  • Supports, instructs and educates the patient, family and caregiver.
  • Documents problems, nursing assessments, appropriate goals, care provided, care interventions and patient and family outcomes.
  • Coordinates all patient and family services with the interdisciplinary group (IDG).
  • Maintains the dignity of the dying patient.
  • Supports the patient’s and family’s unique spiritual and cultural beliefs.
  • Participates in the IDG meeting to include exchange of information, review of problems, and sharing of professional support.
  • Consults and collaborates with the interdisciplinary team and other involved in the patient’s care.
  • Administers medications and treatments according to the Ohio Board of Nursing Nurse Practice Act
  • Performs other activities or tasks within the scope of practice of the registered nurse.
  • Provides holistic, family-centered care across treatment setting to improve the quality of life.
  • Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community
  • Participates in the Quality Assessment Performance Improvement (QAPI) program
  • Demonstrates flexibility in meeting the needs of the program and responds positively to changes in workflow.
  • Complies with agency infection control and safety policies.
  • Completes annual competency requirements.
  • Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel.
  • Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by Unit Director.
  • Protects patient health information according HIPAA Privacy Rule and Aultman policies.
  • Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation.
  • Supports strategic organizational goals.
  • Abides by the Aultman Code of Conduct at all times.

Working Conditions

  • Meal and break times are coordinated with other staff members to maintain adequate coverage.
  • Shifts are scheduled or changed due to the staffing needs of the unit/department based upon volume and acuity.
  • Work weekends and holidays as assigned or as applicable based on department needs.
  • Cross training depending upon department needs.

Department Working Conditions

  • Hours of operation with shifts as assigned.
  • Subject to frequent interruptions and changes in priority of duties throughout the day.
  • Subject to emergency and other crisis situations
  • Hazardous Exposure Category 1
  • Sitting/standing/moving about during working hours (see attached Physical Requirements Addendum for details)
  • Performing patient care responsibilities as required.

Hazardous Exposure Category

Category I - Includes tasks that involve exposure to blood, body fluids, or tissues. All procedures or other job-related tasks that involve an inherent potential for spills or splashes of, or mucous membrane or skin contact with blood, body fluids or tissues are Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks.
