About The Role

We are seeking a compassionate and highly organised Registered Nurse to join our team as a Part-Time Veteran Care Nurse Coordinator. This unique hybrid role combines clinical care coordination, telehealth patient screening, and in-clinic nursing consultations, with a strong focus on supporting veterans and their ongoing healthcare needs.

Working closely with GPs and allied health professionals, you will play a key role in delivering person-centred care, improving patient outcomes, and ensuring continuity of care across a multidisciplinary environment. This part-time position offers a combination of remote and in-clinic work, providing flexibility while allowing you to make a meaningful impact on the lives of veterans and their families.

What You'll Own
  • Coordinate and manage ongoing care for veterans and other patients, ensuring continuity of care and regular wellbeing follow-ups.
  • Conduct telehealth pre-screening consultations, patient intake assessments, and comprehensive medical history reviews.
  • Prepare patient files and clinical information for GP review and assessment.
  • Identify clinical risks, contraindications, medication interactions, and escalation requirements.
  • Conduct face-to-face nursing consultations and intake assessments for new clinic patients.
  • Support chronic disease management, preventative health initiatives, and veteran health reviews.
  • Monitor patient progress and escalate clinical concerns where appropriate.
  • Liaise with GPs, specialists, allied health providers, pharmacies, hospitals, and community support services.
  • Assist with hospital admission and discharge coordination when required.
  • Build strong therapeutic relationships with veterans and their families, providing advocacy and ongoing support.
  • Identify psychosocial, mental health, and support service needs, facilitating appropriate referrals and care pathways.
  • Educate patients on treatment pathways, care plans, and available support services.
  • Assist with patient recalls, follow-up scheduling, and continuity of care initiatives.
  • Maintain accurate and timely clinical records in accordance with AHPRA, Medicare, DVA, privacy, and clinical governance requirements.
  • Participate in quality improvement initiatives, clinical audits, and ongoing professional development activities.
What You'll Need
  • Current unrestricted AHPRA registration as a Registered Nurse (RN).
  • Minimum 2 years of post-registration clinical nursing experience.
  • Experience in care coordination, chronic disease management, primary care, telehealth, or community nursing.
  • Strong clinical assessment, patient engagement, and documentation skills.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse patient groups.
  • Ability to work autonomously while collaborating effectively within a multidisciplinary healthcare team.
  • Experience using clinical software systems and telehealth platforms.
  • Strong organisational, time management, and problem-solving skills.
  • A patient-centred approach and genuine passion for improving health outcomes.
Highly Regarded
  • Experience working with veterans or veteran-focused healthcare services.
  • Care coordination experience within primary care settings.
  • Mental health, chronic pain management, or complex medication management experience.
  • Knowledge of Medicare and DVA processes.
  • Rural, regional, or outreach nursing experience.
  • Current Driver's Licence.
