Start date of December 2026.

Under the direction of the primary care physician, provide health maintenance as well as care to patients with common acute and chronic illness. The Advanced Practice Nurse or Physician's Assistant is responsible for directing a patient’s care that is comprehensive, continuous, and coordinated. The Advanced Practice Nurse is responsible for health assessment, early detection of illness, health promotion, and disease prevention. Collaboration with the patient’s primary care/personal physician and referral to other health care providers, patient advocacy, teaching and family counseling are major components of the responsibilities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Practices within professional scope and current standards to assess, diagnose, and treat patients with acute and chronic conditions.
  • Obtains complete medical history and physical data on patients.
  • Maintains and reviews patient’s records, charts, and other pertinent information.
  • Collaborates with physicians in managing acute and long-term medical needs of patients when necessary.
  • Provide quality care for patients of all ages.
  • Recognizes deviations from normal health; records and formulates an appropriate treatment plan.
  • Counsel’s patients and their family regarding diet, growth and development, social habits, and routine health care.
  • Orders appropriate laboratory and diagnostic tests.
  • Interprets data to determine further diagnostic and therapeutic procedures as needed.
  • Administers medications and injections if needed.
  • Instructs patient and family regarding medications and treatment instructions.
  • Interviews and advises patients regarding health and illness prevention.
  • Recommends community resources to meet patient and family needs.
  • Possesses the knowledge of professional nursing theory, practice, and medical care to provide and evaluate care.
  • Possesses knowledge of medical equipment and instruments to administer patient care.
  • Applies and modifies the principles, methods, and techniques of professional patient care.
  • Participates in professional development activities and maintains professional affiliations.
  • Maintains patient confidentiality.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to interpret, adapt and apply guidelines and procedures.
  • Arranges for and/or refers patients to needed services that cannot be provided at the clinic.
  • Assures that adequate patient health records are maintained and transferred as required when patients are referred.
  • Participates with the physician in a periodic review of patient’s health records.
  • Attends/completes any necessary continuing education.
  • Participates in all safety programs which may include assignment to an emergency response team.
  • Performs other duties as assigned by Manager.

OTHER RESPONSIBILITIES

  • Adheres to policies and procedures addressing the issue of pain management. Understands the patient's right to pain management. Recognizes and responds appropriately.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Receives training and/or attends necessary meetings to meet the criteria as outlined in the Hospital’s Corporate Compliance Plan and Code of Conduct. Understands the responsibilities related to compliance and knows how to contact the Corporate Compliance Officer should there be any instance of question or concern regarding fraud and/or abuse.

Requirements:

EDUCATION AND/OR EXPERIENCE

  • Completion of an accredited Nurse Practitioner and/or meets the Advanced Nursing Practice requirements set forth by the state of Illinois.
  • 2-5 years of experience preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

  • Possess and maintains licensing as required by the State of Illinois for APN.
  • Possess and maintains DEA license.
  • CPR certification or willing to obtain within 30 days of hire.

PHYSICAL DEMANDS

  • While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; reach with hands and arms; talk or hear; and taste or smell.
  • The employee is occasionally required to walk, sit; climb or balance; and stoop, kneel, crouch, or crawl. The employee must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 100 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, and ability to adjust focus.

WORK ENVIRONMENT

  • While performing the duties of this job, the employee is occasionally exposed to fumes or airborne particles and toxic or caustic chemicals.
  • May be exposed to infectious diseases.
  • May be exposed to combative patients.
  • The noise level in the work environment is usually moderate.
