About the Role:

As a Licensed Practical Nurse (LPN) at Concordia Senior Living, you will play a crucial role in providing high-quality care to our residents in assisted living and memory care settings. Your primary responsibility will be to support the health and well-being of our residents by administering medications, monitoring vital signs, and assisting with daily living activities. You will collaborate closely with registered nurses and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each resident. Additionally, you will be responsible for maintaining accurate medical records and ensuring compliance with all healthcare regulations and standards. Ultimately, your compassionate care and attention to detail will significantly enhance the quality of life for our residents and their families.

Minimum Qualifications:

  • Current LPN license in the state of practice.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.

Preferred Qualifications:

  • Experience in geriatric nursing or long-term care settings.
  • Certification in dementia care or related fields.
  • Strong communication and interpersonal skills.

Responsibilities:

  • Administer medications and treatments as prescribed by physicians.
  • Monitor and document residents' health status and vital signs.
  • Assist residents with daily living activities, including bathing, dressing, and grooming.
  • Collaborate with healthcare team members to develop and implement care plans.
  • Provide emotional support and companionship to residents and their families.
