The Registered Nurse (RN) provides comprehensive skilled nursing care to medically complex patients in accordance with physician orders, the individualized Plan of Care, and Aspen Healthcare policies.
The RN is responsible for performing comprehensive assessments, developing and updating nursing plans of care, educating patients and families, and coordinating care with physicians and the interdisciplinary healthcare team.
Assignments may include care provided in:
- Private Homes
- Hospitals
- Skilled Nursing Facilities
- Assisted Living Communities
- Other Healthcare Settings as Assigned
- Perform comprehensive nursing assessments and develop individualized Plans of Care.
- Deliver high-quality one-on-one skilled nursing care in accordance with physician orders.
- Provide specialized nursing care for medically complex and respiratory-compromised patients, including the management of tracheostomy and ventilator-dependent patients.
- Perform comprehensive respiratory assessments and manage oxygen therapy, ventilator settings (within agency protocols), tracheostomy care, airway management, suctioning, and respiratory monitoring.
- Recognize early signs of respiratory distress and respond appropriately during emergencies.
- Administer medications, IV therapies (when applicable), injections, and physician-ordered treatments.
- Perform wound assessments and advanced wound care.
- Manage feeding tubes (G-Tube/NG Tube), urinary catheters, central lines (when applicable), and other skilled nursing procedures.
- Monitor patient progress and revise the nursing Plan of Care as needed.
- Collaborate with physicians regarding changes in patient condition and obtain additional orders when appropriate.
- Supervise and provide clinical oversight for LVNs, caregivers, and other assigned clinical staff in accordance with Texas Board of Nursing regulations.
- Educate patients and family members regarding disease management, medications, respiratory equipment, emergency preparedness, infection prevention, and home safety.
- Coordinate care with physicians, respiratory therapists, therapists, case managers, caregivers, and the interdisciplinary healthcare team.
- Maintain accurate, timely, and complete clinical documentation.
- Ensure compliance with Medicare, Medicaid, state, federal, OSHA, HIPAA, and Aspen Healthcare policies.
- Participate in case conferences, quality improvement initiatives, annual competencies, and required in-service education.

Requirements: Minimum Qualifications- Current Registered Nurse (RN) license in the State of Texas.
- Current CPR/BLS Certification.
- ACLS certification preferred.
- Minimum one (1) year of nursing experience preferred.
- Previous experience in Private Duty Nursing, Home Health, ICU, Critical Care, Pediatrics, Long-Term Care, Rehabilitation, or Hospital Nursing preferred.
- Experience caring for tracheostomy, ventilator-dependent, respiratory-compromised, or medically fragile patients is highly desirable.
- Valid Texas Driver's License.
- Reliable transportation.
- Current automobile insurance.
- Ability to travel throughout the Sherman and Denison service area.

Candidates must be able to:
- Stand, walk, bend, kneel, lift, push, and pull up to 50 pounds with or without reasonable accommodation.

- Perform prolonged periods of direct patient care.
- Safely operate medical equipment and patient care devices.
