Position Overview

Under the direct supervision of the Care Coordination Outreach Supervisor, the Care Coordination Outreach Worker / IBCCP supports the appropriate delivery of healthcare services to Aunt Martha’s patients.

This position enrolls eligible patients in the Illinois Breast and Cervical Cancer Program (IBCCP), coordinates medical follow-up for infants requiring additional screening, completes health-risk assessments, and provides outreach and referrals for medium- and high-risk patients.

The role also schedules follow-up appointments for patients recently discharged from an inpatient hospital stay or emergency department visit. The ideal candidate is organized, dependable, comfortable communicating with patients, and able to manage multiple priorities while maintaining accuracy and attention to detail.

Key ResponsibilitiesIBCCP Enrollment and Patient Support
  • Provide appropriate services to new and established patients who may be eligible for the IBCCP program.
  • Assist patients with determining IBCCP eligibility and completing the enrollment process.
  • Collaborate with clinic staff, care coordination staff, other Aunt Martha’s programs, and external organizations to reach eligible patients.
  • Support efforts to achieve the program’s assigned outreach and enrollment goals.
  • Provide patients with information about screenings, preventive services, and available healthcare resources.
Patient Outreach and Follow-Up
  • Conduct outreach to active care coordination patients through clinic, home, and hospital visits as required.
  • Complete health-risk screenings for assigned patients.
  • Schedule appointments for patients recently discharged from inpatient care or an emergency department.
  • Coordinate medical follow-up for infants requiring additional screening or services.
  • Provide referrals and follow-up support for medium- and high-risk patients.
  • Educate patients about appropriate emergency department use and preventive healthcare services.
Care Coordination and Collaboration
  • Collaborate with health plans, specialists, hospitals, physicians, clinic staff, and community partners to support coordinated patient care.
  • Help patients access screenings, appointments, referrals, and follow-up services.
  • Communicate patient needs, barriers, and outcomes to appropriate members of the care team.
  • Work independently and as part of a collaborative team to meet program goals and priorities.
Documentation and Administrative Support
  • Accurately document patient information, outreach activities, referrals, and outcomes in the Athena Electronic Health Record.
  • Maintain program databases and outcomes information.
  • Answer assigned telephone calls professionally, respond to inquiries, route communications, and transcribe messages.
  • Maintain patient confidentiality in accordance with HIPAA, organizational policies, and applicable laws.
  • Attend required meetings and trainings.
  • Complete all required Core Curriculum training.
  • Perform other duties as assigned.
QualificationsEducation and Experience
  • High School Diploma or GED preferred; no college degree required.
  • Experience in healthcare, community outreach, patient services, customer service, or care coordination is preferred.
  • Bilingual English/Spanish proficiency is preferred.
Skills and Competencies
  • Strong verbal and written communication skills.
  • Proficiency with Microsoft Office programs.
  • Comfortable learning and using electronic health records and program databases.
  • Strong customer service and interpersonal skills.
  • Ability to organize and manage multiple priorities.

  • Ability to work 100% on-site at the Park Forest location.
  • Ability to maintain patient confidentiality and professional boundaries.
  • Ability to conduct outreach in clinic, hospital, home, or community settings as required by the program.
  • Ability to attend all required meetings and training sessions.
