General Description

Sharing their own personal experience of recovery and using a trauma-informed, non-clinical approach. The HCBS Peer Navigator will engage in activities to assist individuals on the HCBS Waiver in initiating recovery, maintaining recovery and enhancing the quality of personal and family life in long-term recovery.

Essential Duties and Responsibilities

Advocacy:

• Assists individuals in seeking and obtaining benefits and entitlements, food, shelter, permanent housing, etc.
• Assists individuals with linkage to and system navigation within behavioral health and allied human services systems to access appropriate care.
• Assists individuals in advocating for self-directed services.

Outreach and Engagement:

• Assists individuals in companionship and modeling of recovery lifestyle, including participation in recovery activities that might be beyond the scope of treatment providers (e.g. recovery celebration event, attending a social event, etc.)
• Raises the awareness of existing services, pathways to recovery and helping individual in removing barriers that may be blocking access to his/her recovery.

Self-help tools:

• Assists individuals in selecting and utilizing self-directed recovery tools such as Relapse Prevention Planning and things that bring a sense of purpose and meaning into his/her life.
• Assists individuals in connecting to natural supports that enhance the quality and security of life.
• Assists individuals in making connections to self-help groups in the community.

Recovery Supports:

• Provides recovery education and coaching for individuals on the waiver and their family members.
• Offers one to one peer support.
• Assists the individual in assimilating into his/her community.

Transitional Supports:

• Provides a bridge for an individual leaving jail, prison, institution or general hospital and moving into a residential setting.
• Assists the individual in assimilating into his/her community.

Pre-crisis and Crisis Supports:

• Provides companionship when an individual is in an emergency room or crisis unit or preparing to be admitted to detox, residential or other services to deal with crisis.
• Provides peer support in the individual’s home or in the community to support them before (or in) a crisis or relapse.
• Develops crisis diversion or relapse prevention plans.

General:

• Maintains accurate documentation of all services provided to individuals in accordance with standards set by funding sources in the Electronic Health Record database.
• Participates in team meetings, case conferences, and case reviews.
• Participates in staff, department, and agency meetings or activities.
• Represents Catholic Charities of Livingston County on appropriate boards and committees as assigned by supervisor.
• Assures compliance with all applicable local, State and Federal regulations and agency policies.
• Attends mandatory training.
• Completes other duties as assigned.

Qualifications

Education: High School Diploma or GED

Credentials: Peer Specialist Certification or Recovery Peer Advocate Certification is required

Experience: At least one year of relevant experience working with substance use disorders and/or serious illness Must have knowledge of behavioral health system and resources Must have familiarity and sensitivity to mental health and substance use challenges Bilingual (Spanish/English) is desirable Relevant years of experience and education will be considered

Additional Qualifications:

• Ability to prioritize assignments, plan, and complete work projects with minimal direction
• Possess a valid NYS Driver’s license and a driver’s record considered acceptable by agency and insurance carrier
• Continuous use of a reliable, registered and insured vehicle
• Ability to travel in all weather conditions throughout the counties served.
• Ability to multi-task and prioritize duties.
• Excellent verbal/written skills
• Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations
• Ability to maintain confidentiality and handle confidential information with discretion
• Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies
• Ability to work in a cooperative and helpful manner with all individuals
• Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
• Provide effective, equitable, understandable and respectful quality care and services that are responsive to diverse cultural health beliefs and practices, preferred languages, health literacy and other communication needs.
