We are seeking a compassionate and patient-focused Family Medicine Physician to join our team on a part-time basis. Position Responsibilities
- Provide comprehensive primary care services to pediatric, adolescent, adult, and geriatric patients.
- Deliver compassionate, patient-centered care to individuals with intellectual and developmental disabilities (I/DD), serious mental illness (SMI), behavioral health conditions, and substance use disorders.
- Collaborate with behavioral health and care management teams to support patients with complex medical, psychological, and social needs.
- Diagnose and treat acute and chronic medical conditions.
- Develop individualized treatment plans and coordinate care across disciplines.
- Order, review, and interpret diagnostic and laboratory tests.
- Prescribe medications and monitor treatment effectiveness.
- Perform office-based procedures as appropriate.
- Maintain accurate and timely electronic medical records.
- Participate in an integrated care model focused on improving health outcomes for vulnerable and underserved populations.
Qualifications
Required
- MD or DO from an accredited medical school
- Completion of an accredited Family Medicine residency program
- Active New York State Physician License
- Current DEA registration (or ability to obtain)
- Experience providing care in an outpatient setting
Preferred
- Experience working with individuals with intellectual and developmental disabilities (I/DD), serious mental illness (SMI), behavioral health conditions, and/or substance use disorders.
- Experience in a Federally Qualified Health Center (FQHC), community health center, or underserved setting.
- Bilingual in Spanish and English.
