Clinical Case Manager – Medical Assistant/LPN
Job Summary:
Georgia Mountains Health is a Federally Qualified Health Care (FQHC), Primary Care facility that is mission driven to provide access to care by providing professional, compassionate and affordable, quality healthcare to our communities. We are looking for a Clinical Case Worker for an immediate full-time position in the Dahlonega location, work schedule of 8a-5p, M-F, no nights, no weekends and holidays off.
The Case Manager will be part of the primary care team helping to resolve patient issues that could affect smooth care progression by addressing their medical and social determinants of health needs. The Case Manager will coordinate medical and other services for individuals and use Medical Assistant/LPN skills to assist providers as needed. This position requires excellent customer service skills and a working knowledge of medical terminology and medical office procedures. Experience in primary care/family health experience highly preferred. Experience with electronic medical records, Athena preferred.
Key Responsibilities include but not limited to:
- Community networking to connect patients with essential resources.
- Serve as a liaison between patients, healthcare providers, and other agencies.
- Provides information for basic services, application assistance, and care planning for patients, as needed.
- Assist patients with referral coordination and appointment scheduling.
- Monitor treatment progress and modify care plans as needed to optimize outcomes.
- Insurance enrollment and assist patients with ongoing eligibility in medical/insurance programs.
- Coordinating information, gathering and reporting daily, weekly and/or monthly patient statics.
- Document all activities and maintain accurate, up-to-date case notes in compliance with regulations.
- Providing education to patients and families to ensure understanding and effective communication.
- Assists providers with physical examinations by preparing treatment/exam room with proper supplies, instruments, and materials, as directed.
- Performs routine laboratory tests
- Perform vital signs, EKG, phlebotomy, and document in patient’s medical record.
- Communicate independently and effectively with medical providers.

Requirements:
- High school diploma or equivalent.
- Medical Assistant/Certified Medical Assistant
- Active BLS certification
- 1-2 years of relevant previous experience.
- Strong knowledge of community health resources and assistance programs.
- Satisfactory background check and drug test required.
