About This Position
Job Summary
The Case Manager will provide case management services for clients with intellectual and developmental disabilities.
Essential Duties & Responsibilities
- Provide habilitative, behavioral and health care coordination services to clients with intellectual and developmental disabilities (IDD) as part of a multi-disciplinary ISP team.
- Complete timely and accurate documentation within Therap and other documentation as required of the position.
- Assess, plan, implement, and document care and habilitative coordination services to assigned clients
- Ensure the ISP expectations are fulfilled and the ISP is correct. Ensure the appropriate personnel are aware of the ISP expectations.
- Complete regular file reviews to ensure compliance with the ISP and regulatory requirements
- Participate in staffing meetings and consultation with other clinicians to assess care coordination needs of clients and encourage appropriate referrals to internal and external community services.
- Act as an advocate to clients to ensure they receive necessary services in the community.
- Develop knowledge and establish good working relationships with community support resources and services within the community.
- Maintain communication and positive multi-disciplinary working relationships with service coordinators and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.
- Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills, specifically as it relates to clients with intellectual and developmental disabilities.
- Other duties as assigned
Education & Experience
- Bachelor's degree in human services or related field preferred
- At least one year of experience with IDD population
- Valid driver's license
- Current automobile insurance
- Ability to pass registry and background checks

Essential Functions/Physical Requirements
Good hearing and visual skills; ability to bend, stoop, sit, stand, reach, and lift up to 25 pounds.
