About This Position
We are seeking a passionate and proactive Community Liaison and Care Coordinator to join our team in Placer County (Job is in Placer, home office is in Irvine, CA). The ideal candidate will be instrumental in recruiting patients eligible for ECM services—specifically those with high hospital or emergency department utilization, individuals being discharged from skilled nursing facilities, and those at risk of long-term institutionalization. This role involves extensive community outreach, patient engagement, and ongoing care coordination for up to one year per patient.
Key Responsibilities:
Patient Recruitment:
Identify and engage potential patients eligible for ECM services.
Collaborate with hospitals, nursing homes, churches, community organizations, and doctor offices to source referrals.
Educate case managers and healthcare professionals about ECM eligibility criteria and referral processes.
Community Outreach:
Serve as a liaison between our organization and community partners.
Build and maintain strong relationships with healthcare providers, community leaders, and organizations.
Represent the organization at community events, meetings, and forums.
Care Coordination:
Provide monthly care coordination for patients for up to one year.
Facilitate referrals to community support services based on patient needs.
Coordinate care plans with patients' primary care physicians (PCPs) and other healthcare providers.
Monitor patient progress and adjust care plans as necessary.
Documentation & Reporting:
Maintain accurate and confidential records of patient interactions and care plans.
Prepare regular reports on outreach activities, patient recruitment, and care coordination outcomes.
Qualifications:
Experience:
Proven experience as an in-person marketer or community liaison.
Demonstrated ability to work effectively with community partners, healthcare professionals, patients, and families.
Skills & Abilities:
Exceptional interpersonal and communication skills.
Comfortable speaking publicly and engaging with diverse groups.
Strong organizational and time-management skills.
Ability to work independently and collaboratively within a team.
Proficiency in basic computer applications (e.g., Microsoft Office Suite).
Requirements:
Must possess a reliable vehicle and a valid driver's license.
Willingness to travel locally within Placer County.
Knowledge of local community resources and healthcare landscape is highly desirable.
What We Offer:
Competitive hourly wage of $25/hour. Options for Pay Increase: $27/hour after reaching and maintaining 30 clients on panel. $30/hour after reaching and maintaining 45 clients on panel.
Other Pay: Mileage reimbursement is at a rate of 70 cents per mile from office to client location. This rate may change, as it is based on the IRS mileage reimbursement rate.
Benefits: Benefits are available: Health insurance, dental, vision, 401K, short term disability, long term disability benefits are offered where Employee pays anywhere from 50-100% of the premium benefit costs.
Working Hours: Full Time (40 hours per week), 9AM - 1PM and 2PM - 6PM. (Lunch: 1 - 2PM)
Time Off: Vacation and sick time hours are accrued. They are eligible for use after a waiting period. Full time employees have ten (10) paid holidays (Thanksgiving Day, the day after Thanksgiving, Christmas Eve, Christmas Day, New Year’s Day, 4th of July, Labor Day, Memorial Day, Juneteenth and Martin Luther King’s Birthday).
