About This Position
Position Description:
In this role, youll serve as the primary point of contact for community agencies, coordinate aftercare referrals, and schedule follow-up appointments. Youll assess needs for benefits, medications, and housing, assisting with applications and ensuring continuity of care. Youll also help individuals navigate legal requirements, such as future court dates and registration obligations, and provide individual and group interventions to support successful reintegration. Throughout, youll demonstrate cultural and age-specific competency while collaborating with treatment teams to promote long-term success.What Were Looking For Education: Bachelors degree in social work, counseling, or a related field (required) Experience: Two (2) years of relevant experience (preferred) Why Join Us
