Job Title: Family Care Plan Coordinator

Program/Department: Women and Family Referral Center (WFRC)

Position Type: Full-Time 40 hours/ week – Hybrid Position (remote, on-site, varied locations statewide)

Location: Statewide, MA

Reports to: Director of Recovery

About IHR: The Institute for Health and Recovery (IHR) is a statewide service, research, policy, program development and capacity building agency. IHR designs its services based on an understanding of the impact of trauma. IHR’s CARF accredited treatment programs provide holistic interventions for hard-to-reach and high-risk individuals and families. IHR’s mission, since our founding in 1990, has been to develop and support a comprehensive continuum of care for families, individuals, youth, and pregnant and parenting people affected by alcohol, tobacco, and other drug use, violence/trauma, mental health challenges and other health issues, while advancing principles of health equity and social justice. Our work is focused on addressing and eradicating inequities in our health care, economic, and justice systems, especially as they disproportionately affect women, fathers and people of color. We are committed to providing services and creating communities that mitigate the effects of substance use, mental illness, and trauma.

Summary Statement: The Family Care Plan Coordinator supports pregnant and parenting people in developing and implementing plans to support their own recovery and the safety and wellbeing of their child(ren). The Family Care Plan Coordinator cultivates strong relationships with providers, convenes and facilitates Family Care Plan meetings, and provides ongoing care coordination, navigation, and flexible support for each individual and family.

Essential Functions:
  • Actively engage pregnant and parenting people in person and virtually to offer support and share information about Family Care Planning.
  • Support participants in assessing their own needs and priorities as well as the needs of their children.
  • Work collaboratively with Perinatal Peer Recovery Specialist for each participant to develop dynamic, individualized Family Care Plans that reflect their values, priorities, and goals.
  • Connect participants with services, supports, and material, community, and informational resources, following up to ensure access, suitability, and satisfaction.
  • Educate providers, individuals, and community members about the purpose of family care planning and how plans may be used across systems.
  • Assist participants in identifying people, services, and resources to include in their Family Care Plans.
  • Facilitate participant-led planning that promotes safety, healing, and stability.
  • Maintain regular contact with participants including in person, telephonic, and video communications in accordance with their preferences and needs.
  • With participant consent, work with providers across systems to integrate Family Care Plans into broader care coordination efforts.
  • Monitor, review, and update the Family Care Plan to reflect the changing needs of the individual and family.
  • Provide ongoing navigation, support, advocacy, and linkages to care throughout the plan period.
  • Maintain timely records of all services providers in EHR system in accordance with state and federal confidentiality laws. 
  • Use cloud-based systems for communication with IHR’s site office, including shared calendars, timesheets, and supervisor check-ins.  
  • Attend team and agency staff meetings.
Qualifications/Requirements:
  • Lived or professional experience with pregnancy and parenting of infants with prenatal substance exposure.
  • Bachelor’s Degree in human services or professional experience equivalent, and at least 5 years’ experience coordinating care
  • Understanding of the Massachusetts treatment system and recovery communities and supports.
  • Familiarity with child and family services and systems, including child welfare, home visiting, Early Intervention, etc.
  • Exceptional communication and facilitation skills.
  • Fluency in English and at least one other language: (Spanish, Portuguese, or Haitian Creole preferred)
  • Strong understanding of child development and attachment. 
  • Understanding of multiple recovery pathways and commitment to supporting each individual in developing a plan that meets their needs and preferences.
  • Skills in Motivational Interviewing, and in strengths-based, trauma-informed service delivery.
