Job Description:
As a Home Health Nurse at Custom, you'll play a crucial role in empowering our patients to regain independence and improve their quality of life. Your responsibilities will include:

- Conducting thorough initial assessments to develop personalized care plans in collaboration with physicians and the interdisciplinary team.

Qualifications:
- Valid registered nursing license (required).
- 1+ years of registered nursing experience.
