We are seeking an experienced Home Health Physical Therapist (PT) to serve patients throughout Washington, DC. This role offers clinical autonomy while working alongside a collaborative interdisciplinary team.

• Perform comprehensive physical therapy evaluations
  • Develop and implement individualized treatment plans
  • Provide skilled therapy to improve mobility, balance, strength, and safety
  • Educate patients and caregivers on home exercise programs
  • Complete accurate and timely EMR documentation
  • Collaborate with nurses, physicians, and care team members
  • Supervise PTAs as needed
  • Ensure compliance with Medicare, HIPAA, and agency policies

• Graduate of an accredited Physical Therapy program
  • Active DC Physical Therapist license (Required)
  • Minimum 3 years clinical experience (Home Health preferred)
  • OASIS experience strongly preferred
  • EMR experience (WellSky/Kinnser or Homecare Homebase preferred)
  • Strong clinical judgment and communication skills
  • Valid driver’s license & reliable transportation
