About This Role:The Physical Therapist at Evara Health provides patient-centered care to help individuals improve mobility, restore function, and manage musculoskeletal and neuromuscular conditions. Working as part of a multidisciplinary team, this role develops individualized treatment plans and helps patients achieve their goals and improve their quality of life.**This posting is being used to build a talent pipeline for future opportunities at Evara Health. What You'll do:
- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Provide therapeutic interventions, including exercises, manual therapy, and patient education.
- Monitor patient progress and adjust treatment plans to support optimal outcomes.
- Educate patients and families on treatment plans, exercises, and strategies to support ongoing health and mobility.
- Collaborate with providers and other healthcare professionals to ensure coordinated, patient-centered care.
- Maintain accurate and timely patient documentation in accordance with professional and regulatory standards.
- Stay current with physical therapy best practices and incorporate evidence-based care.
- Collaborate with and mentor residents, students, and trainees while supporting clinical education and staff development.
- Provide high-quality, comprehensive care while promoting cost-effective practices.
- Maintain compliance with Evara Health policies and all applicable HIPAA, HRSA, state, federal, and accreditation requirements.

Required Education & Experience:
- Doctor of Physical Therapy (DPT) degree from an accredited institution.
- A minimum of two years of professional experience in physical therapy, demonstrating a broad range of clinical skills in a healthcare setting.
- Exceptional communication and interpersonal skills to effectively engage with patients, families, and healthcare teams.
