Job Description

Description:

The Community Health Programs Manager oversees culturally responsive health, prevention, and behavioral health initiatives and federally funded programs. This position is responsible for program administration, grant compliance, community outreach, and coordination of prevention-focused services and activities that support the health and wellness of the Snoqualmie Tribal community. The Community HealthPrograms Manager collaborates with internal departments, healthcare providers, and community partners to improve access to culturally grounded health resources and support community health priorities.

Essential Duties:

  • Lead community education, outreach, and promotion efforts related to Snoqualmie SUD and behavioral health programs and services.
  • Oversee program administration, implementation, data collection, and reporting for assigned programs and initiatives.
  • Support program development, prevention initiatives, and continuous quality improvement efforts for SUD and behavioral health services.
  • Maintain confidentiality of client and program information in compliance with HIPAA, 42 CFR Part 2, and applicable policies and regulations.
  • Coordinate training, seminars, and educational activities for staff and community members.
  • Oversee budgets, expenditures, and fiscal monitoring for assigned grants, programs, and contracts to ensure compliance with funding requirements and organizational policies.
  • Develop, implement, and monitor annual workplans to ensure activities, timelines, and expenditures align with approved budgets and program objectives.
  • Monitor program deliverables, outcomes, data quality, and reporting requirements to ensure compliance with funder expectations and support continuous quality improvement.
  • Develop and maintain collaborative partnerships with internal departments, healthcare providers, community organizations, and external agencies.
  • Lead the development, implementation, administration, monitoring, and evaluation of the Tribe’s Smoking Cessation Program, including community outreach, education, data collection, reporting, and compliance with applicable funding requirements.

Additional Duties:

  • Assist the Health & Wellness department with any special projects and assignments as requested.
  • The Tribe values its commitment to community service and the well-being of its members. In recognition of this, each employee may be expected to perform duties from time to time which may not be related to their regular responsibilities.
  • Other duties as assigned.
Requirements:
  • Ability to maintain confidentiality and communicate with tact, professionalism, and discretion.
  • Knowledge of and ability to comply with HIPAA, 42 CFR Part 2, and applicable federal, state, and tribal regulations. Excellent verbal, written, and interpersonal communication skills.
  • Ability to manage multiple projects, priorities, and deadlines effectively.
  • Ability to develop and maintain collaborative working relationships with community members, partner organizations, and internal departments.
  • Knowledge of substance use disorder (SUD), behavioral health, prevention, and community outreach programs.
  • Experience working with Native American communities strongly preferred

Education & Experience:

Education Required:

  • Associate’s degree in Tribal Administration, Public Health, Social Services, Behavioral Health, or a closely related field
  • Four (4) years of relevant experience may substitute for a Bachelors degree, and two (2) years of relevant experience may substitute for an Associate’s degree.

Education Preferred:

  • Bachelors degree in Public Health, Social Services, Behavioral Health, Tribal Administration, or related field.

Experience:

  • Minimum of three (3) years’ of experience working in prevention, behavioral health, intervention, or Tribal Health programs.
  • Minimum of three (3) years of experience with community outreach, health education, prevention initiatives, or public health programming.
  • Minimum of one (1) year of experience coordinating community events, trainings, or educational activities preferred.
  • Minimum of three (3) years of experience with program reporting, data collection, and compliance monitoring preferred.
  • Experience coordinating federally funded programs, grants, or contracts preferred.

Other Requirements:

  • Must be able to pass a thorough pre-employment background investigation.
  • Must agree to be subject to the Tribe's Controlled Substance and Alcohol Testing Policy to include pre-employment screening..

Work Environment:

The work environment includes an office environment in a controlled atmosphere building. This position also includes frequent driving of a motor vehicle and transportation of clients to and from their homes and their appointments.

Tribal Preference:

The Snoqualmie Indian Tribe follows the Tribal Hiring Preference as outlined in tribal policies, including the Tribal Employment Rights Ordinance (TERO). TERO sets forth certain hiring preferences for qualified individuals who are enrolled in a federally recognized Indian tribe, as well as, in some cases, preference for certain family members of those individuals.

Additional Information:

  • Salary, compensation will be determined based on experience and starts at $88,726.03. This position is a regular full-time position and employment is at will.
