Home Health Intake Coordinator in Federal Way, Washington at Choice Rehab

Choice Health at Home is seeking an organized, customer-focused Intake Referral Coordinator to manage the initial patient referral process. You will serve as a key liaison between hospitals, physicians, patients, families, and clinical staff to support a seamless start of care.

Professional experience working with insurance or in a home health office is required.

• Receive, review, and accurately document new referrals in a timely manner.

• Verify insurance eligibility, benefits, and payer sources.

• Obtain, maintain, and track insurance authorizations and initial certifications.

• Accurately enter patient and referral information into the electronic medical record.

• Coordinate with Nursing Supervisors, patients, and the interdisciplinary team to support timely care.

• Communicate effectively with patients, families, staff, healthcare professionals, and referral sources.

• Respond to agency and patient needs professionally, using sound judgment and problem-solving skills.

• Participate in agency training and the Quality Assessment and Performance Improvement (QAPI) program.

• Follow agency policies and support a commitment to quality patient care.

Required:

• Professional experience working with insurance or in a home health office.

• Excellent computer, organizational, and customer service skills.

• Strong communication skills and attention to detail.

• Valid driver’s license and auto insurance.

Preferred:

• At least one year of referral intake experience.

• Experience with third-party payers, Medicare, and Medicaid.

• Medical field background and familiarity with medical terminology.

• HomeCare HomeBase experience.
