Overview of Position
The Part-Time Designated Coordinator is a growth-oriented position beginning at approximately 24 hours per week, with the opportunity to increase to up to 40 hours per week as the agency’s caseload, staffing needs, and program responsibilities increase. The Designated Coordinator is responsible for the onboarding, training, coaching, oversight, and performance evaluation of direct support professionals. This includes setting staff schedules and monitoring and reviewing the quality of supports and services provided under the agency’s 245D license.
The position requires great attention to detail and accuracy, strong organizational and time-management skills, excellent oral and written communication skills, critical thinking, problem-solving skills, collaboration with various teams and individuals, and a person-centered approach when working with the individuals we serve. All duties and oversight must be performed in compliance with applicable 245D requirements and agency policies and procedures.
Nature and Scope:
The Designated Coordinator directly reports to the Designated Manager and, when appropriate, may also report to the Program Director/Owner of the agency. The Designated Coordinator will use a person-centered approach when coordinating and overseeing the wants, needs, preferences, service delivery, and programming of persons served. The Designated Coordinator will implement and monitor support plan addendums and outcomes developed in collaboration with persons served, their support teams, and the Designated Manager. The Designated Coordinator will participate in and help coordinate regular meetings to support compliance with 245D requirements and continuity of care. The Designated Coordinator will communicate with care teams regarding the best ways to support each person served and will invite appropriate parties to meetings as needed to coordinate services.
The job requires frequent contact and collaboration with persons served, families, staff, service providers, community agencies, managed care organizations, DHS, and lead agency/county case managers.
The work environment requires frequent standing, sitting, walking, and taking stairs. A valid driver’s license, current automobile insurance, reliable transportation, and the ability to drive are required. The Designated Coordinator may work in the homes of persons served, at agency-owned ICS properties, in community settings, at the administrative office, and remotely when authorized and appropriate for assigned duties.
Essential Functions of Part-Time Designated Coordinator:
Coordination of person-served programming and services:
• Complete and update required paperwork, assessments, orientations, and service-related documentation for persons served and their caregivers.
• Participate in support team meetings for persons served.
• Use a person-centered approach to assist with the development, implementation, and monitoring of support plan addendums and outcomes.
• Review documentation to support compliance with 245D requirements and agency policies and procedures.
• Maintain a current working knowledge of applicable regulatory and licensing requirements.
• Maintain accurate and timely records, reports, and program documentation.
• Serve as a communication liaison among persons served, legal representatives or guardians, case managers, direct support professionals, and other service providers.
• Review incident and emergency reports, identify patterns or concerns, and assist with implementation of corrective action as appropriate.
• Provide person-centered advocacy for persons served.
• Review person-served files and documentation to ensure required records are complete, current, and maintained appropriately.

Organize and coordinate programming for persons served
• Assist with and oversee the development and coordination of individualized programming for persons served.
• Conduct program evaluations and quality assurance reviews as assigned.

Orientation, Supervision, and Training of Direct Support Staff
• Monitor annual training, compliance, and competency requirements for direct support staff.
• Assist with onboarding, orientation, and training of new staff.
• Conduct regular training, monitoring, coaching, and check-ins with staff.
• Conduct direct observation of direct support staff and provide direction, coaching, and corrective guidance as needed.
• Promote a person-centered, respectful, professional, and positive teamwork environment.
• Support the Designated Manager with employee performance reviews and evaluations.
• Support the Designated Manager with overseeing and enforcing agency policies and procedures.
• Assist with coordinating staff schedules, identifying staffing gaps, and arranging coverage as needed.
• Perform other duties as assigned by the Designated Manager, Program Director, or President/CEO.
Minimum Qualifications:
• 4-year degree and 1 year of full-time experience as a Designated Coordinator
• Or 2-year degree and 2 years of full-time experience as a Designated Coordinator
• Or diploma from a post-secondary school and 3 years of full-time experience as a Designated Coordinator
• Or 50 hours of education/training in the human services field and 4 years of full-time experience as a Designated Coordinator
• Knowledge of Minnesota Health Care Programs, CADI, BI, EW, DD, and related waiver programs and the AC Program preferred.
• Excellent communication and interpersonal skills.
• Excellent critical-thinking, problem-solving, organizational, time-management, and decision-making skills.
• Ability to read, write, and speak the English language to adequately interact with and respond to the needs of persons served, staff, and other stakeholders.
• Ability to use a computer for required documentation, communication, scheduling, training, and timekeeping purposes.
• Ability to use Microsoft Office, Google Workspace, Pavillio, MN-ITS, and other databases and software utilized by the agency.
• Valid driver’s license, current automobile insurance, reliable transportation, and ability to travel between service locations.
• Authorized to work in the United States of America.
• Ability to successfully complete required NetStudy/background study and fingerprinting requirements.
Employment Status: Part-Time, Non-Exempt. The position begins at approximately 24 hours per week and may increase up to 40 hours per week as the workload and agency needs increase.
Salary: $23-$25 per hour
Benefits for full-time employees: 401(k) and employer match after 2 years of employment, PTO, 10 paid holidays, and mileage reimbursement. Flexible work environment and work-from-home options may be available when appropriate and authorized.
Location: St. Paul, Minnesota
Schedule: The position will initially be scheduled for approximately 24 hours per week. As the agency’s caseload, staffing needs, and job responsibilities increase, scheduled hours may gradually increase up to 40 hours per week, with the potential to transition to full-time employment. The work schedule will be based on program needs, persons served, required meetings, employee supervision and training needs, documentation responsibilities, and regulatory requirements. Scheduling flexibility is required and may include daytime, evening, and occasional weekend hours.
Physical Demands: The ability to occasionally lift up to 20 lbs.
